I, the undersigned parent or legal guardian of the student dancer, acknowledge and agree to the following Assumption of Riske for Dansation Dance Academy.

I understand that participation in dance classes and activities involves certain risks, including but no limited to physical injury, illness or emotional distress.

I acknowledge that my child's participation in dance or tumbling classes and activities is voluntary and that I have made a personal decision to allow my child to participate in such activities.

I assume all risks associated with my child's participation in dance or tumbling classes and activities, including but no limited to the risks of physical injury, illness, or emotional distress.

I hereby release, waive, and discharge Dansation Dance Academy and its directors, officers, instructors, employees, and agents from any and all claims, liabilities, and damages arising out of my child's participation in dance or tumbling classes and activities, including by not limited to claims for personal injury, illness, or emotional distress.

I agree to indemnify and hold harmless Dansation Dance Academy and its directors, officers, instructors, employees, and agents from any and all cleaims, liabilities, and damages arising out of my child's participation in dance and tumbling classes and activities.

I understand that by signing this Assumption of Risk Waiver, I am giving up certain legal rights and remedies that may be available to me in the event of an injury, illness, or other harm suffered by my child during dance or tumbling classes and activities.

By signing this Assumption of Risk Waiver, I am acknowledge that I have read and understand the risks associate with participation in dance classes and activities, and that I voluntarily assume all such risks.
