I understand that with any activity, including dance, cheer, and tumbling, there is a possibility for injury. I give permission for my child to participate in the N-Step Dance Academy Program and hereby waive and absolve N-Step Dance Academy and all divisions thereof, of any and all liability and responsibility for injuries, sickness, accidents and/or acts of god incurred during participation in and/or instruction of classes, private coaching, choreography and/or other dance, tumbling, or cheerleading related activities by myself or my child whose name is listed above.
In consideration of my signed release allowing my child/myself to participate in the N-Step Dance Academy Program, I, intending to be legally bound, do hereby, my heirs, executor and administration, waive, release, and forever discharge any and all rights and claims for damage which I may or which may hereafter accrue to me against N-Step Dance Academy, the program, program directors or the respective employees, office agents, representatives, successors, and/or assignees, for any and all damages which may be sustained by me or my child in connection with my association with or participation in, or riding out of travel to and/or from the respective N-Step Dance Academy activities and classes. In the event of injury/accident/sickness, N-Step Dance Academy officials and/or instructors are to contact the designated adult listed below. In addition, I hereby give my permission for myself/my child to be photographed videotaped and audiotaped to be used in print or broadcast media as deemed appropriate for the promotion of N-Step Dance Academy activities, and for publicity surrounding participation in N-Step Dance Academy events and dance programs.
In addition, I have adequate medical coverage to take care of any injury/sickness/accident/ disability my child may incur. If serious injury occurs that requires immediate medical attention, I give my permission to N-Step Dance Academy staff to transport my child to the hospital until I can be contacted.
