Although I understand that The Dance School will make every reasonable effort to provide a safe environment, I am fully aware and understand that physical activities such as dance involves a certain amount of risk. I acknowledge the possibility that despite precautions, accidents or physical injury to me/my child may occur. I fully accept and assume all such risk and all responsibility for the losses, cost and damages I incur as a result of my participation in activities. I hereby consent to myself and/or my child's participation in programs at The Dance School. I agree to release and hold The Dance School, its Instructors, Administrative staff, Board of Directors and any other agents harmless from any cause of action, claims or demands now and in the future whether known, unknown, anticipated or unanticipated. I will not hold The Dance School liable for any personal injury or any personal property damage or loss which may occur on the premises or while using the facility or its equipment before during or after class, or which may occur at any event sponsored by The Dance School outside the studio, including transportation to or from any event taking place inside or outisde of the studio. Outside events/performance venues include Historic Everett Theater, Everett PUD, Everett Performing Arts Center and Everett Civic Auditorium. Personal medical insurance is the responsibility of each student (family).
General School Rules: Listen politely and follow directions. Keep hands, feet, and objects to yourself. Be courteous and polite. Respect the rights of others as well as the property of The Dance School. Use appropriate language. Expectations of Dancers: Students will arrive to class on time, will be dressed in appropriate attire, and have hair properly secured. Students will respectfully listen to their instructors and administrators. Students under the age of 18 will ask for assistance from the adults in charge when they have trouble with another student. The Dance School strives to provide a safe and positive environment for all. We have a no tolerance policy for bullying of any kind. Students who have been accused of bullying and their parents/guardian will be brought in for a meeting with the director. If the situation continues, the student/family will be removed from all programs at TDS.
For dancer's performing in our Year End Performance in June, once a costume is ordered for you and/or your dancer you are responsible for the full costume fee that is due. If you or your dancer withdraws from TDS prior to the performance, the costume fee is due at time of withdrawal - once the costume arrives it will be available for pickup during office hours. Once the costume has been ordered The Dance School has incurred the cost of the costume and cannot return it. Exchanges can be made to exchange costume sizes and exchange defective costumes only.
Inclement weather may result in school closure, make-up classes may be added at the end of the term. The Dance School follows the Everett Public School's closure policies due to inclement weather for our morning classes (classes schedule before 3:00 pm). Please note that evening classes may be held if weather conditions improve. Everett Public Schools may announce an altered schedule or closure due to weather, however, often times the ice or snow will melt by the time our evening classes begin. The best way to find out if we are having evening classes it to check your email for an announcement, check our Facebook and Instagram accounts for latest announcements and/or the website front page. You will not receive a phone call in the event of a Snow Day. We will do our very best to keep families updated. Of course, not everyone lives near the main roads so regardless of our decision to continue evening classes, we recommend doing what is best and safest for your family. If classes are cancelled due to inclement weather The Dance School will either schedule a makeup class or credit your account for the cancelled class.
In the event of an accident or illness, I understand that every reasonable effort will be made to contact the parent/guardian immediately. However, if I am unavailable, I authorize The Dance School staff to secure emergency medical care for my child(ren) as needed. Extended absences due to medical reasons or injury require a doctor's note. Students are encouraged to come and observe classes if they are injured and able to attend class. Refunds for extended medical absences will be evaluated on an individual basis.
I hereby authorize The Dance School to use photographs and/or video of myself or my child(ren) in promotional materials (brochures, flyers, posters, social media and website) at their discretion. This includes photo/video taken during class in the studio, dress rehearsals and/or at performances at outside venues.
All personal information given to The Dance School is kept private and is not sold, or distributed outside of the school.
I understand and acknowledge that I am aware this facility located at 2821 Rockefeller Ave, Everett, WA 98201 also known as The Dance School uses 24-hour video and audio monitoring and surveillance. This monitoring and surveillance system is used throughout the interior and exterior of the facility for insurance and liability purposes.
