I, AS A PARTICIPANT OR PARENT/GUARDIAN OF SAID CHILD(REN) HEREBY ASSUME ALL OF THE RISKS OF MYSELF/MY CHILD(REN) PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH IN-PERSON AND IN-PERSON AND VIRTUAL DANCE/YOGA/MUSIC CLASSES, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective reigns as follows:

At Cary-Grove Performing Arts Centre, we cannot guarantee that your child will not be exposed (whether at Cary-Grove Performing Arts Centre or elsewhere) to COVID-19 or any other contagious illness. By typing your name and initialing below you agree to waive any claim of liability against Cary-Grove Performing Arts Centre and to hold Cary-Grove Performing Arts Centre harmless if your child contracts COVID-19 despite our best efforts to prevent it.

Additionally, I agree to notify Cary-Grove Performing Arts Centre if my child or any member of our household contracts COVID-19 and follow current guidelines for isolation/masking from the CDC. This action will help ensure the utmost safety for our dancers and staff. I furthermore agree to have my student take class via Zoom and will not send my student to Cary-Grove Performing Arts Centre in person if they are known to be sick.

I understand and agree that by participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury and, in rare circumstances, death. I agree to release and hold harmless Cary-Grove Performing Arts Centre, including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold Cary-Grove Performing Arts Centre liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I understand that I should be aware of my, and/or my child's physical limitations and agree not to exceed them.

I understand that classes taught via virtual learning (Zoom or other platform) will present a situation where the instructor is not physically in the room with the participant, and the responsibility lies with the participant to provide a safe space in which to take class. Also, for children, a parent should always be present as the instructor cannot properly supervise via video-conferencing instruction.

Further, I authorize and agree that Cary-Grove Performing Arts Centre may take and use photographs/videos of me and/or my child for purposes of record-keeping, advertising, and marketing. I understand that I do not have any rights to these photographs/videos and will not be compensated for the same. I understand that Cary-Grove Performing Arts Centre cannot be responsible for what the parents of classmates film and if my child is in their video or photos. If for any reason my student cannot have their likeness shared in video or photograph, I will specifically discuss this with the CGPAC staff and indicate it in the child's DSP account so we can communicate this need with this student's teacher(s).

If I am signing this waiver for my child(ren), I certify that I am the parent or legal guardian and have the right to waive these rights.

I understand that this waiver will have a starting date and will remain in effect until the family no longer participates in any programs at Cary-Grove Performing Arts Centre.
