I am not aware of any injury, illnesses, or other health related issues that would restrict or limit my child(ren)'s ability to participate in dance/physical activity. I agree to assume all risks and expenses due to any injury that may occur as a result in any of K&M related dance activities. I agree to hold K&M or anyone acting on behalf of K&M Studio harmless in the event of an injury to my child(ren) while participating under the supervision of the above. K&M Studio has my permission to use my child(ren)'s photograph/video publicly. I understand that the images may be used in print publications, online publications, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use. If I do not wish for my child(ren)'s photograph to be released, I will notify the studio.

Tuition will be automatically taken from my default payment account on the 1st of every month. If a payment has not been made by the 7th of the month, a $25 late fee will be applied. If planning to withdraw from a class, the studio must be notified at least two weeks prior to the 1st of the month to qualify for reimbursement. If the studio has not been notified before this timeframe, your card will be charged and there will be no refunds issued. If a payment method is declined, a new method is required on the portal. If all online payment methods fail, a cash or certified bank check is required within 7 days of the month to avoid a late fee.
