WAIVER OF LIABILITY
NOTICE: BY SIGNING THIS DOCUMENT YOU GIVE UP IMPORTANT LEGAL RIGHTS.
READ THIS CAREFULLY BEFORE YOU SIGN.
In consideration of being allowed to participate in the dance classes and other programming of Juxtaposition Studios LLC, a North Carolina limited liability company (“Juxtaposition Studiosâ€):
1. I, , do hereby fully and forever release, discharge and hold harmless, Juxtaposition Studios, its owners, agents and employees from any and all claims for property damage or personal injury that may arise from my participation in dance classes at Juxtaposition Studios or my use of the equipment and facilities provided by Juxtaposition Studios, including, but not limited to, any and all claims arising from the negligence or carelessness of Juxtaposition Studios, its owners, agents and employees.
2. By signing this document I fully understand that dancing is physically demanding (even to persons in the very best of physical condition) and that my participation in this activity may lead to physical injury or even death. I hereby certify that I am in good physical condition and that I am unaware of any health related problems that may affect my ability to engage in the dancing and other activities provided by Juxtaposition Studios.
3. By signing this document I fully understand that there are risks of physical injury associated with, arising out of and inherent in the activity of dance. I assume all risks of participation in this endeavor. I am fully aware that conditions may exist, foreseeable and unforeseeable, that might pose a risk to me of physical injury or even death. I certify that all information provided to Juxtaposition Studios, including without limitation, my age and dancing experience, is accurate and given freely for the purpose of participating in this activity.
4. By signing this document, I certify that my participation in this event and my signature on this document is completely voluntarily and that I am acting of my own free will. I have not used nor am I under the influence of alcohol or any other drug. I fully understand that I do not have to participate in this activity, that I may cease to participate at any time and that I am free to participate in other activities of this type elsewhere.
5. I hereby agree that any legal disputes regarding the interpretation of this document will be determined by the substantive laws of the State of North Carolina. I also understand that I am free to seek the advice of independent legal counsel, at my own expense, prior to signing this document; and that any advice, suggestions or statements made by other participants or by anyone else, including the owners, agents or employees of Juxtaposition Studios may be incorrect and unsupported by the law.
6. Photo Release: I give full rights to Juxtaposition Studios and its owners, agents and employees to use photos and video images of me or my child to use for promotional purposes of Juxtaposition Studios. Photos and video may be used in brochures, websites, advertisements, and other promotional material created by Juxtaposition Studios. Photos may appear with or without names in press releases and other print advertising.
7. Medical Release: I give permission for any and all medical attention to be administered to me or my child in the event of accident, injury, sickness, etc., under the direction of the physician listed below or at any necessary emergency facility, until such time as I may be contacted. I also assume responsibility for the payment of any such treatment.
I HAVE READ THIS DOCUMENT AND I AM STILL WILLING TO SIGN IT. I FULLY UNDERSTAND THAT BY SIGNING THIS DOCUMENT I GIVE UP MANY IMPORTANT LEGAL RIGHTS THAT MAY OTHERWISE BE AVAILABLE TO ME.

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of Juxtaposition Studios, its owners, agents and employees, and, for myself, my heirs, assigns, and next of kin. I release and agree to indemnify and hold harmless Juxtaposition Studios, its owners, agents and employees from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM NEGLIGENCE OR CARELESSNESS, to the fullest extent permitted by law. The participant has my permission to participate in Juxtaposition Studios classes and activities.

8. Clients must submit a 30-day notice to Juxtaposition Studios LLC/Juxtaposition Fine Arts via email: juxtaposition.fine.arts@gmail.com prior to dropping any classes. You will be pro-rated and charged for the classes if you drop prior to 30 days. If 30-day notice in not given, no refunds will be given.
9. There is no lobby provided to wait during classes. All classes are independent unless specifically stated. Parents/Guardians/Grown-ups/Nannies/Sitters/Siblings/etc. are not allowed to stay in the classrooms during classes. This helps provide a calm and safe environment for all of the students and instructors.
10. Showcase 2024 is NOT a requirement. IF participating you will be billed a $30 Performance Fee which includes: Costume rental and cleaning fee. This will be billed to your account on April 1, 2024. Showcase 2024 will be May 31-June 2, 2024 at Reynolds Place. Schedule is as follows:

7:30PM Show 1: Big Kids (Ages 11 and up, private lessons, CODA)
SATURDAY:
10AM Show 2: Sunday & Monday Classes
2PM Show 3: Tuesday & Wednesday Classes
6PM Show 4: Thursday & Saturday Classes
SUNDAY:
1PM Show 5: Big Kids (Ages 11 and up, private lessons, CODA)
4PM Show 6: Sensory Friendly Show (those who wish to participate may sign-up for this showcase)
11. Tuition: Auto-Pay is run on the 1st of each month (pro-rated for Aug 21-31, 2023). September- May: Monthly Tuition. June & July-Flex Summer Classes.
