I hereby state that all participant are in good mental and physical health condition to participate in the activities provided by Vault Elite Cheer LLC, including but not limited to all aspects of cheerleading, tumbling, and dance trainning. I am fully aware that any activity involving motion, height or athletic activity creates the possibility of serious injury. I hereby release Vault Elite Cheer LLC, it's employees and staff from liability to the above named athlete occurring in the premises of Vault Elite Cheer.
I understand that Vault Elite Cheer has the right to deny admittance to any student of the program as it sees fit. I also agree not to hold these parties responsible in the event that my son/daughter/child engages in inappropriate conduct ( including, but not limited to disruptive or volatile behavior, eat.) or becomes involved in any activity or with any person not associated with Vault Elite Cheer LLC, or its scheduled program and that Vault Elite Cheer LLC has the right to send him/her home for inappropriate conduct. I further attest that the information contained in this application is correct to the best of my knowledge. in addition, I have agreed to the policy and fee statement and agree to comply.
I authorize all medical and surgical treatment, X-ray, laboratory, anesthesia, and other medical and/or paramedics for my child and waive my right to informed consent of treatment. This waiver applies only in the event that neither parent/guardian can be reached in the case of an emergency.
I give permission for my child to be included in any media releases of Vault Elite LLC. Such use includes the display, distribution, publication, transmission or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos and digital images.
