From time to time, we hire photographers to take updated pictures for use in publications and promotional materials.
We would never use a photo that would reflect negatively on its subjects. Please read and sign the release below.
I grant to Westwood Ballet/Irish/Visual Art its employees, agents, and assignees, the right and permission to make, use, reuse, and/or publish Photographic pictures of ____________________________(child's name), which may be in connection with his/her own name or a fictitious name, for any purpose whatsoever including primarily any promotional materials or website designed or authorized by
Westwood Ballet/Irish/Visual Arts. I waive any right to inspect or approve the finished photograph or printed matter that may include such pictures.

I understand and agree to the following (initial each)

_________1. Tuition is based on 8-month school year fee schedule but may be paid monthly.

_________2. Tuition payments are due on the first of each month, September through April (Ballet & Visual Art) (Irish 12 months), and
are considered delinquent. If not paid by the 10th of each month a $10 late fee will be charged for each month a payment
is late. There is a $25.00 NSF fee for returned checks.

Students to participate in any Westwood Ballet/Irish/Visual Art year end Performance/Art Show.


_________4. I agree to pay the Ballet costume-performance fee $120 / $75 Irish costume rental-performance fee and
$50 performance fee.

_________5. I understand that Westwood Ballet/Irish/Visual Art reserves the right to cancel any class that does not have a
sufficient enrollment.

In the event that my daughter/son or I (adult) __________________________________ becomes ill or injured during Irish dance, Ballet Dance, or Art class, and neither I nor the emergency contact individuals I listed previously can be reached by telephone, I authorize Cindy Best, or other instructor in charge of class to seek emergency medical assistance and/or to otherwise act in the best interest of my child or myself. The signature of one parent is deemed to be given with the consent and authority of both parents.

If specific instructions need to be followed (for example, your child is diabetic and has a plan for treatment), those instructions must be provided to Westwood Arts no later than the first class attended.

In consideration of being permitted to participate in classes offered by Westwood Baptist Church dba Westwood Ballet (hereinafter “Westwood Arts”), the parent(s) and/or legal guardian(s) of the minor child named below, or the name of the adult participating in class, hereby acknowledge, accept and state the following:

1. Participation in an exercise program of any kind, and specifically a dance program, has inherent risks, including the possibility of serious and permanent injury. I acknowledge that it is not possible for Westwood Arts, nor any instructors, to guarantee absolute safety, and on behalf of my child, or myself (adult), ____________________________, I hereby assume the risks of his/her participation in this program.

2. While recognizing these risks, I attest that my child, or myself, is able to participate in the various physical activities associated with these dance classes, activities, or performances. If I determine that such activities are unsafe, I will promptly notify the instructor and take immediate precautions to protect my child, or myself.

3. I hereby release, waive, discharge and covenant not to sue Westwood Baptist Church dba Westwood Ballet/ Westwood Arts, nor any of the owners, members, directors, instructors, managers, agents, employees, volunteer assistants or others who give recommendations, directions, or instructions on their behalf (hereinafter referred to “Releasees”) from all liability to my child or myself, and our heirs and assigns, for any and all claims, injuries, demands, losses, or damages on account of injuries to my child, or myself, or property arising out of or alleged to be related to or caused by participation in this program, including those alleged to be caused by the negligence of said Releasees. I further agree to indemnify and hold harmless the Releasees for any such claims.
