By registering on this Diversity in Dance Student Portal, I agree to the required waivers and indemnities, as follows:

READ CAREFULLY AND ACCEPT BELOW

I hereby indemnify, release, and forever hold harmless Diversity in Dance, LLC, including its owners, employees, and instructors (collectively, "Diversity in Dance", from any and all claims, liabilities, damages, fees, and the like, arising from any accident, personal injury, death, property loss, or damage sustained by my child(ren)/myself/the minor child(ren) for whom I am a legal guardian ("Participant") while Participant is engaged in activities connected with Diversity in Dance, including, but not limited to, classes, rehearsals, private lessons, tutorials, public events, competitions, volunteer/community activities, performances, or other activities, or that is located on any of the premises owned, leased, or utilized by Diversity in Dance before, during, or after such activities.

I understand that dance and acro activities have inherent risks of injury, and, being fully aware of all risk, I consent to have Participant participate in the classes and/or activities offered by Diversity in Dance, and I accept full responsibility for providing adequate health and accident insurance coverage for the protection of Participant. By accepting and consenting to this statement, I declare that Participant is in good health, with known no undisclosed physical conditions that might prevent participation in strenuous and rigorous dance activities and other training and performance activities OR that I have accepted all risks related to my physical conditions and have made Diversity in Dance aware of such. I understand that the responsibility of managing Participant's physical limitations are mine/ours alone and agree that Diversity in Dance shall not be responsible for Participant exceeding those physical limitations.

Further, I understand and acknowledge that because of the physical nature of dance and acro, there may be physical contact between instructors, teachers, and students during rehearsals, shows, workshops, productions, and especially during dance instruction. I understand that at times for proper instruction and safety, physical contact is required and necessary.

Furthermore, I and Participant agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities and equipment utilized by Diversity in Dance.

Further, I authorize and agree that Diversity In Dance may take and use photographs/videos of me and/or the Participant for purposes including, but not limited to, recordkeeping, advertising, social media, and marketing. I understand that I do not have any rights to these photographs/videos and will not be compensated for the same. I further understand and agree that these photographs and videos may be used after we are no longer actively enrolled at Diversity in Dance.

I have carefully read the contents of this waiver form and fully understand its contents. I understand that this is an assumption of risk and release of liability, and I accept it of my own free will.

I understand that use of this electronic portal and participation in Diversity in Dance classes and activities constitutes my affirmation of this Waiver of Liability without the need for physical signature.

I, on behalf of myself and/or the minor children registered by or on behalf of me ("Participant(s)"), hereby grant and authorize Diversity in Dance, LLC, the right to take, edit, alter, copy, exhibit, publish, distribute, and make use of any and all photos and videos (collectively, "Media") taken of Participant(s) to be used in/for any lawful promotional materials and campaigns, including, but not limited to, newsletters, displays, fliers, social media posts, website content, brochures, advertisements, fundraising letters, reports, press kits, and submissions to journalists, websites, social networking sites, and other print or digital communications.

This authorization extends to all languages, media, formats, and markets, whether known or unknown at the time of this agreement.

This authorization shall continue indefinitely unless I revoke this authorization in writing.

I waive the right to inspect or approve any finished product in which Participant's(s') likeness appears, including written or electronic copy.

I understand and agree that the Media is the product of Diversity in Dance and that I do not have any rights to the Media and will not be compensated for the same. I further understand and agree that these photographs and videos may be used after we are no longer actively enrolled at Diversity in Dance.

I hereby HOLD HARMLESS, RELEASE, and forever DISCHARGE Diversity in Dance, and its owners, operators, staff, employees, and contractors, from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I understand that use of this electronic portal and participation in Diversity in Dance classes and activities constitutes my affirmation of this Photo Release without the need for physical signature.

I, on behalf of myself and/or the minor child(ren) for whom I have the parent or legal guardian (the "Participant(s)") hereby INDEMNIFY, RELEASE, and forever HOLD HARMLESS Diversity in Dance, LLC and its owners, employees, and instructors (collectively, "Diversity in Dance"), from any and all claims, liabilities, damages, fees, and the like, arising from any accident, personal injury, death, property loss, or damage sustained by Participant(s), while engaged in activities connected with Diversity in Dance, including, but not limited to, classes, rehearsals, private lessons, tutorials, public events, conventions, competitions, volunteer/community activities, performances, or other activities, or that is located on any of the premises owned, leased, or utilized by Diversity in Dance before, during, or after such activities (collectively, the "Activities").

I understand that dance and acro activities have inherent risks of injury, and, being fully aware of all risk, I consent to have Participant(s) participate in the Activities offered by Diversity in Dance. I accept full responsibility for providing adequate health and accident insurance coverage for the protection of Participant(s).
