I, the participant and parent or guardian of, do hereby certify that I or my child is in good health and do not have any physical injuries, disabilities, impairments or health problems, which would interfere with mine or my child's participation at Movement & Artistic Dance Expressions and hereby hold harmless Movement & Artistic Dance Expressions LLC, liable for any personal injuries of any description sustained during my child's voluntary participation therein. I also understand that Movement & Artistic Dance Expressions is a hands on studio which allows teachers of record to instruct my child by assisting him or her with hands on professionalism.
