Tuition will be charged on the 1st of the month. A late fee of $35.00 will be billed to your account on the 10th of the month if tuition balance is not paid. An additional fee of $35.00 will be billed on the 15th of the month if balance is not paid.

Participant in any and all Synergy Dance Company's activities is not covered by medical or accidental insurance. Each participant must furnish his or her own personal coverage. Dance and tumbling activities have inherent elements of danger. Parent or participant permission is needed in order to call an ambulance in an emergency situation. As a participant, or parent/guardian of minor participant, I hereby save harmless and indemnify Synergy Dance Company, its trustees, and employees from any responsibility for any accident, injury, or damage that may occur as a result of the participant's acts or omissions. In case of accident or sickness, I consent to emergency medical care by ambulance or hospital personnel.

Policies and Procedures Waiver

I hereby acknowledge that I have received, read, and understood all policies and procedures outlined by Synergy Dance Company. I agree to abide by these policies and procedures at all times during my enrollment or affiliation with the business.

I understand that failure to comply with any of the policies and procedures may result in disciplinary action, up to and including termination of enrollment or affiliation with the business.

I understand that it is my responsibility to seek clarification from appropriate personnel if I do not understand any of the policies and procedures outlined by Synergy Dance Company.

I agree that my enrollment or affiliation with Synergy Dance Company is voluntary and that I have not been coerced or forced into agreeing to these policies and procedures.

I hereby release Synergy Dance Company, its officers, directors, employees, and agents from any and all liability arising out of my enrollment or affiliation with the business and my compliance or non-compliance with its policies and procedures.

This waiver shall remain in effect for the duration of my enrollment or affiliation with Synergy Dance Company.
