En Face Studios has a 30 day cancellation policy. Cancellation notice must be in writing in the form of: Email - En Face Studios is not responsible for emails not received. Cancellation is not complete until approval is received from an En Face Studios staff member. Completed Cancellation of Services Form - available at the studio. Once cancellation of services form is approved, any automatic withdrawal will be removed for 30 days from the approval date. En Face Studios does not prorate tuition for missed classes. En Face Studios does not issue any refunds.

As a parent/legal guardian of Student(s) named above, I hereby grant permission to En Face Studios, LLC to use the image of my student(s) registered for for classes, including online virtual classes. Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my student(s) for use in materials that include, but may not limited to, printed materials such as brochures and newsletters, advertising, videos, and digital images such as those on the En Face Studios, LLC website, Facebook, and Instagram page. By signing this form, I understand that I am giving unrestricted permission for my student's image to be used in print, video, and digital media. I agree that these images may be used by En Face Studios, LLC for a variety of purposes and that these images may be used without further notifying me. I further understand that no financial compensation will be received by me for use of these photos/videos.

I hereby give my permission to trained medical professionals to administer emergency medical treatment to my child, should sickness or accident occur in my absence.

All clients of En Face Studios will be required to keep a credit or debit card on file at all times. All dancers will be placed on auto payment to be directly charged to a debit or credit card on the 1st of each month. Cash or check payment will be accepted if payment is made before the 1st of the month. If the card on file is declined, the account will be charged a $35 fee. If the balance is not paid in its entirety within 7 days of the decline, the dancer will be removed from all classes at En Face Studios, until account is brought current. A late fee of $25.00 will be applied to all delinquent account on the 7th of the month. If an account remains unpaid for 30 days, the appropriate collection agency will be contacted. There will be a $25.00 service charge for all returned checks. It is the responsibility of the client to inform En Face Studios of any expired cards, changes in card numbers, lost or stolen cards, etc. En Face Studios will not relinquish the payment responsibility of the dancer's parent or guardian/s for any reason, nor will they authorize extended late payment plans. Our dance year runs from June to June and tuition remains the same whether it is a long or short month. This includes months that a holiday break is scheduled. We strongly recommend dancers make up any missed classes during scheduled holiday breaks. Should the need arise to pull a dancer from classes for any particular month, their slot in the class is not guaranteed and the process of re-registration will begin upon re-registering for class. En Face Studios does not prorate tuition unless you are a new student starting mid-month. If the studio must be closed due to circumstances beyond our control (power outages, inclement weather, etc) there will be no refunds or make-ups scheduled. En Face Studios does not issue refunds of any kind.

As a parent/legal guardian of student(s) named above, I hereby consent to the above person(s) participating in En Face Studios programs including in studio and all online virtual programs, understanding that participation in such programs can result in physical injury, and therefore assume all risks of any such injury which might occur as a result of such participation in any activity connected with En Face Studios, LLC. I represent to En Face Studios, LLC that my child is medically fit to participate in any and all programs for which we have enrolled, including online virtual programs. As parent/legal guardian of the Student(s), I hereby agree to be responsible for any and all medical expenses which may be incurred by my child as a result of any injury sustained while training at or performing for En Face Studios, LLC including all online virtual programs.
