Medical Waiver
I grant permission to the staff of Sanctuary Dance Academy to take first aid or emergency measures as judged necessary for the care and protection of my child while under the supervision of the studio. In case of medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the emergency unit deems it necessary. I understand that in some medical situations the staff will need to contact the emergency resource before the child’s parent, physician, and or other person acting on the parent’s behalf. I also understand and agree that the child’s parents or legal guardians shall be responsible for any expenses incurred.

I realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I (the dancer over age 18 or parent/guardian) assume all risks related to the use of any and all spaces used by Sanctuary Dance Academy. I agree to release and hold harmless Sanctuary Dance Academy including its owners, teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. Furthermore, acknowledge that I have read the studio policies and I agree to obey the studio and facility rules/policies and take full responsibility for mine and my children/children under my care's behavior in addition to any damage I/we may cause to the facilities utilized by Sanctuary Dance Academy.

In the event that a camp must be canceled due to low enrollment, a full refund will be issued to each student enrolled in the canceled camp.
DANCE CAMP DRESS CODE:
Dress code for dance camps is more relaxed than our regular school year as we understand many of our students may be trying out dance for the first time, and we love that! While we would love for everyone to follow our dress code for ballet if possible (which you can find HERE), stretchy pants or leggings (no shorts), t-shirts, and socks are also acceptable. Shirts should not expose midriff or cleavage and may be tank top, short or long-sleeved (no camisoles, please). Older girls are advised to wear supportive sports bras. Hair must be pulled back from the face in a ponytail for ages 3-9, but must be in a bun for ages 10 and up.
GENERAL STUDIO/GYM POLICIES
Sanctuary Dance Academy currently rents the studio space inside "Shape Up" gym, located at 1436 N. Gateway Avenue, Unit 140, Rockwood, Tn. 37854. To enter the gym, please ring the doorbell and a staff member will gladly open the door for you.
Dress codes are created for a dancer's safety. Students are expected to abide by the dress code while in class.
Students must be supervised by their parent or guardian at all times prior to and after camp. Students younger than 16 should not exit the building alone or wait outside for pick-up.
Parents and siblings are welcome to wait in the gym lobby while dance camp is in session. Young siblings may use the play area. However, please supervise children CLOSELY at all times and do not allow them to play loudly or throw toys. Loud or unruly children will be asked to exit the children's area and sit with their parents.
For your safety, please use the walking track to access the restrooms. Walking through the gym equipment area is not allowed.
Parents or class assistants must accompany students younger than the age of 12 to the bathroom. Older students may go alone or in pairs at the discretion of the teacher or parent.
Only gym members may use the gym equipment at any time. If you would like to join the gym or purchase a day pass for $5, please speak with a gym staff member and they will be happy to assist you!
Parents and siblings may watch through the double doors (which are left slightly ajar for observation) but are not allowed to come inside the studio during camp time as this poses a distraction for the students.
Please try to arrive 5-10 minutes before camp begins.
No food or drink is allowed inside the studio room except bottled water with a secure lid.
Cell phone use is not allowed during class time.
Students and parents must be respectful of teachers, gym staff, and all other people at all times. Failure to do so may result in dismissal from the studio. Disrespectful or threatening words or behavior will not be tolerated.
Sanctuary Dance Academy reserves the right to dismiss a student or family from the studio for any reason at any time without any refunds should it be deemed necessary to do so.

Tuition is due by the 10th of each month; any tuition received later than this date will be charged a $15 late fee. If payments are made late habitually, you will be required to enroll in auto-pay. Please help us meet our financial obligations by paying by the due date. Thank you!
Tuition may be paid via cash, check, credit/debit card or ACH payment (credit/debit card and ACH available online only)
Cash: Please place cash inside an envelope and write the student(s) name on it to ensure the proper account is credited. Please use the exact amount, if possible.

Written notice is required when withdrawing from a class by the first day of the month of withdrawal in order to avoid your account being charged.
Because class fees are averaged out throughout the year, tuition costs remain the same each month regardless of how many class weeks are in the month.

Sanctuary Dance Academy follows Roane County Schools for inclement weather closures. Missed classes will be made-up at a later date or prorated at the director's discretion.
Students that miss an excessive amount of classes in a semester (four or more) may have a smaller part in the spring dance due to the teachers' inability to instruct them in the missed technique training and choreography. A private lesson with the teacher is recommended to help the student catch up. Private lesson rates are $60 per hour.
Required rehearsals must be attended by the dancer in order for them to be allowed to perform in the recital or other performances. No exceptions can be made.
At least one parent is required to volunteer during our spring dress rehearsal. This volunteer position may be either an in-person volunteer time slot or through a volunteer donation (a sign-up list will be sent via email in the spring). While we promise to do our very best to not require any parents to volunteer backstage during the actual recital, we may have need of parent volunteers if we are short-handed and need more adults to help keep all students properly supervised. Thank you for your participation!
