I, the parent/legal guardian of the enrolled child/children, understand that Tracie's School of Dance and its staff will not be held responsible for any illness/injury which may occur while at the studio, at conventions/competitions, or during/at any performances. I also agree to hold harmless Tracie's School of Dance and its staff for any losses, damages, and expenses resulting from such illness/injury.

I acknowledge that tuition is due by the 10th of each month beginning with September and ending in May. There is a $10 late fee that will be automatically added to my account if payment is not made on time.
