I agree that:

- The information provided is complete and correct to the best of my knowledge. I will advise the gym immediately if any changes occur.
- I declare and confirm that I have legal right to give all such information, permissions, consents etc as given in this document for the enrolled child. I will accept any and all liability for this child if this claim is unable to be supported and will not in any way hold BCD responsible.
- Payment for training must be made in accordance with the BCD Payment Policy. Overdue accounts will be charged a late fee. If unpaid in 1 month, accounts will be forwarded onto a debt collection agency and the parent/guardian and athlete are liable for all recovery fees as well as the debt.
- Goods purchased by parents/athletes will only be supplied upon full payment of outstanding accounts.
- No payments made to BCD are refundable (unless at the discretion of the gym owners).
- I acknowledge that BCD prohibits private recordings of any dance/cheerleading display, performance or competition.
- I give permission for this athlete to be photographed/recorded etc while participating in any studio activities. I consent for the photos/videos to be used for publicity if required.
- I acknowledge that it is my responsibility to convey my athlete safely to and from the door of the gym and acknowledge that BCD staff are only responsible for the athlete whilst in the confines of the room in which that dancing/cheerleading instructions are to take place and that the athlete receives instruction from the coaching staff at the risk of the parent/guardian.
- I consent to the athlete receiving dance/cheerleading instructions from the BCD staff and warrant that I am not aware of any health conditions or other reasons which would make it inadvisable for the athlete to receive such instruction and will advise the BCD staff immediately if any such reason comes to my notice.
- I acknowledge that participation in dancing/cheerleading activities may include but is not limited too physical activity such as lifts, tosses, rotations, tumbling and height related activities and involves risks that may result in personal injury and/or property damage and notwithstanding this, the athlete wishes to participate voluntarily and assumes the risk in respect of any injuries sustained and/or any property damage.
- The athlete and I shall comply with all State and Federal laws and policies of BCD relating to health and safety, discrimination and harrassment and agree to maintain a dancing/cheerleading environment free from all forms of discrimination, harrasment, bulling and agressive behaviour. BCD may cancel the enrollment of the offending athlete/parents/guardian and such athlete/parents/guardian may be required to leave the gym. In such case no monies are reimbursable to the athlete or parents/guardian/s.
- I agree that BCD or its Servants and/or Agents will not be responsible for any injury caused to the athlete or the athlete's property arising from any accident or mishap while the athlete is attending BCD classes, activities or other events and herby release and indemnify BCD and their servants and/or agents from all claims, damages or actions, costs or other losses arising from dancing/cheerleading classes, activities and events and the use of BCD facilities whatsoever.
- This agreement is binding and maybe relied upon and pleaded in bar to any action, claim, preceding or suit now or anytime in the future taken by me, my heirs, executors and assigns and is given in consideration of BCD, gym owner/s and Servants and/or Agents of the same permitting the athlete to participate in the dancing/cheerleading activities.
- I have read and understood this Enrollment Application, Gym Etiquette and Gym Information Pack and agree to Terms and Conditions stated there.
