I understand that Annemarie's Dance Centre does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Annemarie's Dance Centre, and I voluntarily assume the risks associated with such participation. I hereby certify that my child is in sufficient physical condition to participate without injury. I further recognize the risk of illness and injury inherent in any dance program, and I waive and release Annemarie’s Dance Centre from any claims, costs, liabilities, expenses or judgments arising out of my child’s participation, except for illness or injury resulting from gross negligence or willful misconduct on the part of Annemarie’s Dance Centre Inc. I am willing to provide a physician’s certificate if desired by Annemarie’s Dance Centre.
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Annemarie’s Dance Centre (“the studioâ€) has put in place preventative measures to reduce the spread of COVID-19; however, the studio cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the studio could increase your risk and your child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to, studio employees, volunteers, and studio participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the studio or participation in studio programming (“Claimsâ€). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions or omissions of the studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any studio program. I have read this release of liability and assumption of risk agreement. I fully understand its terms and understand that I have given up substantial rights by signing it freely and voluntarily without any inducement.
PHOTOGRAPHY/VIDEOGRAPHY Photographs and video may be taken of your child in class and/or during ADC events. Your attendance at Annemarie’s Dance Centre will constitute your irrevocable consent to be photographed, videotaped and recorded for the purpose of advertising and promotion in any media and your waiver of any compensation or permission for such use. I hereby execute and deliver this waiver and payment agreement form to permit my child’s participation in Annemarie’s Dance Centre’s programs. I also agree to abide by all the conditions as stated above.
TUITION AND COSTUME PAYMENT AGREEMENT The following dates are when tuition payments are due. Parents/guardian wishing to withdraw their child(ren) must notify Annemarie’s Dance Centre, In writing, at least two (2) weeks before the beginning of the next billing period. Otherwise, the parent will be responsible for tuition payment.
2024-2025 Season September 2024 through May 2025
1st Quarter - Due at time of Registration
2nd Quarter - October 30th 2024
3rd Quarter - January 15th 2025
4th Quarter - March 19th 2025
COSTUME BILLING (FOR 2024 - 2025 SEASON, NOT FOR SUMMER SESSIONS): The studio will end the season with a recital end of April/beginning of May. Costume fees are $70.00 - $80.00 per child per costume and will be charged to your account. Tights, accessories and reasonable alterations are included. There will be no further costume charge. Please note the recital is not mandatory. Annemarie’s Dance Centre must be notified, in writing, before costumes are ordered if your child is not participating in the recital. There are no refunds for costume charges once the costume order is in under any circumstances which includes but is not limited to a shut down due to COVID-19.
COSTUME INFORMATION
Costumes will be charged in October 2024 with second quarter tuition, measured for and ordered in November 2024. The studio makes every effort to select costumes of the highest quality and value. Age appropriate choices are our top priority and we do allow for growth. Costumes and sizes are carefully chosen according to the companies’ size charts. However, costumes are not custom fit and may require alterations. The studio takes care of minor alterations in house at no additional charge. If needed, tights and headpiece are included at no additional charge.
FINANCIAL OBLIGATION
Costumes must be paid in full by October 30th 2024. Costume fees are $70.00 per costume for our combination classes (ages 3 – 7) and $80.00 per costume for level 1 and up. Costumes are not discounted. There will be no further costume charge. If your child is not going to participate in their recital please notify office no later than October 30th 2024. Once costumes are ordered there are no refunds for any reason including but not limited to a shut down due to a pandemic, illness and injury.
DISTRIBUTION
Costumes will be tried on and handed out in the order they arrive to the studio. Distribution will start upon arrival and continue through the end of April or until all costumes are handed out. An email will notify each class when their costumes arrive. Costumes will then be taken home that day. An alteration week will be set for April.
RESPONSIBILITY
Once the costumes are handed out it is the responsibility of the student and family to keep the costume in a safe place. No replacements will be available for lost costumes or items such as headpieces. Please do not put the costume on at home for any reason. Losing or ruining a costume may result in the student’s dismissal from the number.
ATTENDANCE
Beginning in January the recital choreography will be learned. Consistent practice is needed to confidently take the stage. It is important for students to attend class regularly. Excessive absences (4 or more) negatively affects the rest of the class and may result in a student not participating in the recital. It is at the discretion of the teacher whether the situation warrants a dismissal from performing.
I understand that there are no refunds on costume payments or recital tickets once purchased. I also agree to abide by all the conditions as stated above.
