I hereby release Passion Dance Club & Studio or any of its officers, employees, agents, and affiliates from any and all liability in the event of property damage, personal injuries or claims arising from or its connection with my child's participation in Passion Dance Club & Studio programs, including claims that are known or unknown, foreseen and unforeseen, future or contingent.

Passion Dance Club & Studio uses your information for contact, billing, and student information purposes only. We will not share or sell your information with any outside entity or agency. Custodial restrictions are included in this policy and must be addressed on a case-by-case basis.
