I agree that all the information I am submitting is as close to actuate as to my knowledge. I agree to include with this enrollment application my one time registration fee. When it comes to tuition; I understand that payments must be on time in order to continue enrollment in all classes and if I am more then 7 days late for the monthly plan, a fifteen dollar late fee will be applied and attendance to all classes will be suspended until outstanding balance is paid. I also understand that I must pay a costume fee before the given due date to receive costumes. Unpaid costume(s) will not be ordered and participation in the end of the year performance will not be allowed.

I understand that dress attire is mandatory and each class I am are enrolled in will need the proper footwear it requires (tap shoes, ballet slippers, sneakers etc.). If proper attire is not met, after 2 warnings; attendance will be suspended until proper dance attire is present. I understand I am responsible for providing proper dance
