An inherent risk of exposure to COVID-19 exists in any public place where people are present. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by mere presence within this establishment and that such exposure may results in personal injury, illness, or even death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, program participants, and their families. I hereby release Elan Dance Arts from any and all claims arising from or in connection with any direct COVID-19 impact.
Parent's responsibility to be aware of dates and events: it is the responsibility of the parents/adult student to be aware of all studio activities such as viewing days, recitals, extra classes, and dates that the studio is open or closed. The studio will post all such notices on social media as well as email out the info. It is the parent's/adult student's responsibility to regularly check these boards to ensure they are informed. We also have a calendar that everyone can see at the studio as well as take pictures of for easy access. It is their responsibility to inform the studio of any address, phone number or email changes.
Dance Dress Code: required dancewear and dance shoes must be worn to all classes. Failure to wear the required dancewear to class could result in students being asked to sit out for the class. Repeated failure to wear required dancewear could result in termination of classes. Students are not permitted to wear jeans, pajamas, or shoes worn on the street or outdoors, to any dance class. Hair must be tied back. Dance Class Attendance and Lateness: the school reserves the right to have students who come late to class sit out for the class. Students missing more than 4 classes after February might not be allowed to participate in the year end recital. Recital fees still remain non-refundable. The studio should be contacted to be made aware of any absences.
Injuries: parents, legal guardians of minor students and adult students waive the right to any legal action for any injuries sustained on studio property resulting from dance activity or any other activity by the students before, during, or after class time. Please remember safety first while participating in class without an instructor present. Make sure you have parental permission, a safe space to move around in, and do not attempt anything that you do not feel 100% comfortable with. By participating in online classes you are agreeing to waive any and all liability to Elan Dance Arts and its employees. There are no refunds for online classes.
NSF Payments: There will be a $45 plus hst NSF fee for any NSF cheques and a $20 plus hst fee for declined monthly pre-authorized payments. Any late payments are subject to a $20 late fee. Our system does not notify us of expired cards. That is the customers responsibility. Any payments that are declined as a result of an expired card will still be subject to regular late fees. *Monthly Payments are a financing option for full year classes and is not a "Month-to-Month" class option. *if paying by cheque, you must hand in post-dated cheques for the entire session at registration time. *if paying by cash/debit, full payment must be handed in before the 1st of each month otherwise will result in a $20 plus tax late fee. Any debit or cash customers must pay first and last month upon registration.
Photo/Video Release: In signing this form, I hereby authorize Elan Dance Arts to use photo/video of my child for any promotional purposes. If you do not agree, a separate form can be filled out at the studio office.
Registration and recital fees are included in your fees if your program has a recital All families receive 2 free recital tickets. This is 2 tickets per family and not per student or per show. DANCE CARD students DO NOT receive 2 free tickets.
Extreme Weather or Unexpected Interruption of Class: the school may cancel classes due to extreme weather or events beyond our control such as power outages. No refunds will be given for classes missed due to these reasons. Substitutions: the school reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unavailable to teach classes. If a teacher is ill and the studio cannot arrange a substitute, any missed classes will be made up.
Withdrawal and Refunds: All requests for refunds/withdrawal must be done in person and will not be accepted over the phone or email. Refunds and withdrawal requests must be done at the studio office and not with the teacher. *There will be a $35 plus tax cancellation fee for each class registered in if they decide to withdraw. *full year students are eligible for a full refund before they've attended 4 classes. This refund does not include the registration fee and classes attended will be charged for. A withdrawal form must be filled out and handed in before the student's attendance of their 4th class. *a withdrawal form must be filled out and submitted in person at the studio to be eligible for any refund. *withdrawals prior to December 1st will be charged for tuition until December 31st of each year. There are no refunds after December 1st. *The recital fee is non refundable after December 1st. *free classes count as classes attended for the classes that they register for. *monthly pre-authorized payments will not be cancelled until a withdrawal form is submitted to the office. Any intentional declined payments will still be owed with associated declined payment fees. *refunds must be picked up in person at the studio. *there are NO refunds for any summer classes or camps. *there are no refunds for any fees paid during priority registration *there are no refunds for the fitness challenges/food plans *there are no refunds for adult dance cards *there are no refunds for any 6-10 week programs (which includes registration for all 3 sessions of any Preschool Stars/Fine Arts Club programs) *there are no refunds for extenuating circumstances such as COVID-19 or other circumstances that require studio closures Terminations: the studio reserves the right to terminate lessons to any students without notice. Depending on the case a refund for unused lessons may be given. TV advertisers: Every advertiser is on a minimum 3 month contract. There's an additional $20 late fee subject to advertisers who miss a month of fees. The contract does not renew after 3 months, but 3 months must be paid at a minimum.
