ENROLLMENT CONTRACT:

FINANCIAL AGREEMENT:
I agree to pay all payments as indicated. I understand should my child discontinue dancing, refunds will not be given. I understand that the 1st payment must be made in full once I receive the class confirmation. I also agree to be bound by the Rhythm Dance Center "Agreement and Waiver " and state that all information I have provided on the Rhythm Dance Center "2024/25 Online Registration" is accurate and complete.

2024/25 FALL DANCE SESSION: (September until June)
45-minute class* - $520
30-minute class* - $370
*Classes are held once a week, except holidays and scheduled breaks (see the RDC Handbook emailed in the Fall for these dates).

$15 Registration Fee (per family) will be applied to all accounts and is due when registering.

Payment Options:
Option 1 - Payment in Full (due when registering).
Option 2 - Two Installment Plan (due when registering and on 1/1/2024).
Option 3 - Ten Installment Plan (10% of the total amount). The 1st installment is due when registering, and the 2nd through 10th installments are due on the FIRST OF EACH MONTH (September through May).
Note: All installments must be paid in order for the dancer to participate in recitals.

All accounts will need a credit card on file. If payment is not received by the 9th day of the month, the credit card on file will be charged on the 10th day of the month. A $25 fee will be placed on your account for declined credit cards.

GENERAL NOTES:
Payments are accepted in the form of cash, check or credit cards.
Payments may be made in person or mailed to the address listed above.

DISCOUNT STRUCTURE:
(Based on number of classes enrolled per family)
• 2 classes – 5% discount
• 3 or more classes – 10% discount

FINANCIAL AGREEMENT:

I understand the credit card provided on file will be charged on the 10th of the month. I understand should my child discontinue dancing, refunds will not be given. I understand that payment must be made in full no later than May 10, 2024. I also agree to be bound by the Rhythm Dance Center "Agreement and Waiver " and state that all information I have provided on the Rhythm Dance Center "2024/45 Online Registration" is accurate and complete.

I UNDERSTAND that I am registering my child(ren) (individually or collectively) on a monthly basis and that I am free to withdraw the student(s) from the Rhythm Dance Center, LLC ("RDC") program at the end of any month. I agree that it is my responsibility to make payment according to the payment option in the Rhythm Dance Center Enrollment Contract.

I UNDERSTAND that the student's teacher is present, prepared and being paid to teach, whether or not the student is present. I acknowledge that RDC has reserved a place for the student. I understand that I am financially responsible for all installments due for classes missed by the student.

I HEREBY CONSENT that the photographs pertaining to dance activity, matter, advertisement, or programs in which above listed student(s) participated may be used by RDC, their successors or assigns, in whatever way it may desire, including audiovisual productions and television; furthermore, I hereby consent that such photographs and the plates from which they are made shall be RDC's property, and RDC shall have the right to sell, duplicate, reproduce and make other uses of such photographs and plates as RDC may desire free and clear of any claims whatsoever on the student's part, or on behalf of the student.

IN SIGNING THIS DOCUMENT, I fully recognize if anyone is hurt or property damaged while the student is participating in any RDC program or activity (regardless of location and whether participation is in-person or virtual, including travel) neither I, nor any other person on behalf of myself or the student, will have any right to make a claim or file a lawsuit against RDC or any of its members, teachers, employees, or volunteers.

IN SIGNING THIS DOCUMENT, I also acknowledge that I have received and fully recognize, accept, and agree to be bound by all of the terms of the "RDC Team COVID-19 Related Guidelines" and agree to follow all CDC and other government health guidelines to ensure the safety of myself, my student, and others. I understand and agree that participation in any/all RDC programs and activities is at my own risk. If anyone becomes ill from COVID-19 or any other illness while the student is participating in any RDC program or activity (including travel) neither I, nor any other person on behalf of myself or the student, will have any right to make a claim or file a lawsuit against RDC or any of its members, teachers, employees, or volunteers.

IN CONSIDERATION FOR RDC permitting the students to participate in lessons, I hereby release and forever discharge RDC and its members, teachers, employees and volunteers from any and all liability arising out of the student's participation in dance lessons and other activities in connection with said lessons, including, but not limited to, travel to and from events sponsored by or participated in by RDC.

I CERTIFY that the student has no physical or medical condition, other than those listed on the Registration Form, which, to my knowledge, would endanger the student or others if the student participates in dance lessons, or would interfere with the student's ability to participate. I recognize that these lessons may be strenuous and physically demanding and I hereby attest that the student is physically fit and sufficiently trained for participation in these lessons. I am fully aware of the dangers and risks inherent in this type of activity and expressly assume the risk of injury to the student in connection therewith.

I FURTHER CERTIFY that I have read and fully understand this Agreement. I have voluntarily consented to the terms of this Agreement and if I am signing on behalf of my child(ren), I have full authority to do so and all releases and waivers apply to my child(ren) as well as myself.
