Caledonia School of Performing Arts, LLC. Is not responsible for providing before or after class care for students. Students must be promptly picked up after class. Students may not leave class without permission. For safety reasons students must remain inside the studio reception until a parent or guardian arrives to pick them up. Students are not permitted to wait outside in the parking lot or on the side walks.

I, the undersigned, hereby acknowledge that I have read this policy in its entirety and understand all the terms and conditions it contains. For safety reasons appropriate dance wear and dance shoes, as mandated by the Creative Director, must be worn to all classes. Failure to do so may result in injury to student therefor Caledonia School of Performing Arts, LLC. reserves the right to ask student to sit out and observe the class. For safety reasons students are only permitted to wear small studded earrings, absolutely no other jewellery is permitted in class.

For Ballet and Acro classes hair must be in a neat bun. Students unable to wear a bun are permitted to have their hair clipped or tied away from the face. For all other classes hair must be tied back away from the face. This is for the safety of students.

Parents and legal guardians of minor students, and all adult students, waive the right to any legal action for any injury sustained on school property, resulting from normal dance activity or any other activity conducted by the student before, during or after class.

Medical Emergency: the undersigned gives permission to Caledonia School of Performing Arts, LLC. Its owners, directors, teachers and / or representatives, to seek medical treatment for the student in the event they are not able to reach the parent of guardian.

I hereby declare any physical or mental conditions, restrictions, disabilities or additional needs of the student have been stated on the registration form. If not, I hereby declare the student(s) to be in good physical and mental health and fully able to participate, without restrictions, in the activities and instructions at Caledonia School of Performing Arts, LLC.

I, the undersigned, hereby acknowledge that I have read this policy in its entirety and understand all the terms and conditions it contains. By placing my electronic signature, I hereby agree to the above stated policy and execute this agreement voluntarily and with full knowledge of its significance.

Unless otherwise stated I, the undersigned, grant permission for Caledonia School of Performing Arts, LLC. to take photographs and videos of the student(s) to be used for including but not limited to brochures, websites, advertisements and other promotional materials the studio creates. Permission is also herby granted for Caledonia School of Performing Arts, LLC. to copyright such photographs in its name.

I, the undersigned, hereby acknowledge that I have read this policy in its entirety and understand all the terms and conditions it contains. By placing my electronic signature, I hereby agree to the above stated policy and execute this agreement voluntarily and with full knowledge of its significance.

Registration: Caledonia School of Performing Arts, LLC. requires a non-refundable registration fee of $25 per student ($15 for 2nd student) for all new students wishing to register.

Tuition: School tuition is based on a 48 week per annum calendar dived into 12 months. Allowing for a maximum of 4 weeks teacher vacation and training days throughout the calendar year. Tuition is due on or before the 1st of each month. Direct debits will be redeemed on the 1st working day of each month. NO REFUNDS WILL BE GIVEN FOR ANY REASON.

Students with accounts 7 days past due will incur a $5.00 LATE FEE. Accounts that are 14 days past will incur an additional $15.00 LATE FEE. Late fees will NOT be waived under any circumstances. Any student account that is 30 days past due will be inactivated and the student(s) will be removed from all classes. In order to re-activate the account must be paid up to date, including all past due tuition and late fees. All returned checks or insufficient funds transactions will be charged the maximum fee allowed by the law.

If Caledonia School of Performing Arts, LLC. must cancel classes due to events beyond the school’s control including but not limited to, extreme weather or power outages, we will reschedule the missed lessons. NO refunds for lessons missed due to these reasons will be given. If the regularly scheduled teacher is ill or otherwise unable to teach class Caledonia School of Performing Arts, LLC. reserves the right to provide a substitute teacher. If Caledonia School of Performing Arts, LLC. cannot arrange a substitute, any classes missed will be made up. At the discretion of the Creative Director Caledonia School of Performing Arts, LLC. reserves the right to cancel or combine any / all classes.

I have read the tuition policy and understand that I am responsible for tuition payments as described in this policy. Should Caledonia School of Performing Arts, LLC be required to bring legal action to collect under this contract, the prevailing party shall be due reasonable attorneys fees and costs.

I, the undersigned, hereby acknowledge that I have read this policy in its entirety and understand all the terms and conditions it contains. By placing my electronic signature, I hereby agree to the above stated policy and execute this agreement voluntarily and with full knowledge of its significance.

I understand that dance and performing arts training constitutes athletic and physical activities and that injuries may occur. I am willing and able to accept full responsibility for student(s) safety and well-being. I understand that good dance training may involve physical touching and adjustment of the student(s) body by the class teacher / creative director and I give permission for the teacher / creative director to correct students body by physical touch.

I release Caledonia School of Performing Arts, LLC., all employees and all contracted workers from all claims, demands, actions and judgements which the undersigned may have, or claim to have in the future, for all personal injuries, known or unknown, and injuries to property, real or personal, caused by, or arising out of, the activities participated in by my child(ren)/or by me (an adult student) while attending Caledonia School of Performing Arts LLC. run dance studio or while traveling to or attending events associated with Caledonia School of Performing Arts LLC. Included but not limited to dance competitions and theatre performances.

Upon enrolment at Caledonia School of Performing Arts, LLC. Permission must be asked before attending summer schools, conventions or extra classes outwith the studio.

Caledonia School of Performing Arts, LLC. has a list of approved summer schools, conventions and intensive's deemed suitable for students.

I, the undersigned, hereby acknowledge that I have read this policy in its entirety and understand all the terms and conditions it contains. By placing my electronic signature, I hereby agree to the above stated policy and execute this agreement voluntarily and with full knowledge of its significance.

Caledonia School of Performing Arts, LLC. requires a 30 day notice to discontinue any classes or membership. Parent or legal guardian of any student under the age of 18 will be required to complete and sign a withdrawal form provided by Caledonia School of Performing Arts, LLC.

Upon enrolment, through to the conclusion of the withdrawal notice period, the parent or legal guardian of the student will be responsible for the payment of the agreed tuition costs.

All automatic bank debiting or credit card charges will stop after the 30 day notice period. Caledonia School of Performing Arts, LLC. reserves the right to terminate lessons to any student without notice. In such cases, a refund for unused lessons will be given.
