I understand as the parent/guardian/paying member, that monthly membership for classes held at Artistry in Motion Dance Company will be due by the 1st of every month and will be processed with a stored debit/credit card or ACH payment method on file. If I would like to pay on a different date or in another form, it may be paid in advance of the 1st using a check or another debit/credit card. If payment fails, a late fee of $40.00 will be posted after the 10th. No cash is accepted. I understand as the parent/guardian/paying member, that monthly membership is on-going and my account will be continued to be billed every month until I have canceled my or my child's membership. To cancel membership or withdraw from a class, I understand I must give written notice by the 15th of the month prior. If I withdraw after the 15th, I will still be billed for the following month of services. I understand as the parent/guardian/paying member, that all youth dancers enrolled in an on-going weekly class are automatically enrolled in the annual Holiday Show & Recital as a part of their dance education and experience. Production fees - include the show experience, costume, dress rehearsals if needed, performance, and a show gift - and will be processed for all students on September 1 and February 1 of each year the member remains enrolled. Please see THE EXPERIENCE - PERFORMANCE HANDBOOK for specific performance dates and fees. Any dancer who will be out of town or unable to participate must give written notice to Artistry in Motion Dance Company by the 15th of the month prior (August 15, and January 15) or their accounts will be billed. I understand as the parent/guardian/paying member, all monies paid to Artistry in Motion Dance Company for services rendered are non-refundable. Any refunds for unworn dance wear or event or program cancellations can only be issued within 30 days of original payment. Dancers may schedule make-up classes for any missed classes.
