Powerhouse Dance Complex Liability Waiver

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Powerhouse Dance Complex.

I/we agree to release and hold harmless Powerhouse Dance Complex including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Powerhouse Dance Complex liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Powerhouse Dance Complex

I understand that Powerhouse Dance Complex is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.

I/we agree to allow and give permission to Powerhouse Dance Complex to use any photos or video footage of my child/children on the Powerhouse Dance Complex website, Social Media sites or any promotional materials for the purpose of marketing and advertising of Powerhouse Dance Complex.

Your privacy is very important to Powerhouse Dance Comlex. Accordingly, we have developed this Privacy Policy in order for you to understand how we collect, use, communicate, disclose and make use of personal information. The following outlines our privacy policy.

Before or at the time of collecting personal information, we will identify the purposes for which information is being collected.



We will collect and use personal information solely with the objective of fulfilling those purposes specified by us and for other compatible purposes, unless we obtain the consent of the individual concerned or as required by law.



We will only retain personal information as long as necessary for the fulfillment of those purposes.



We will collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the individual concerned.

Tuition Guidelines
Payments are due on the 1st of each month from August through June.

If paying monthly, tuition must be paid by automatic bank account debit or automatic credit card payment. Tuition will be debited from your bank account on approximately the 5th day of each month from September through June. No refunds or prorates are given for tuition.

Add/Withdrawal

Please note that official forms are available at the front desk for all changes to your schedule. No changes will be made to a schedule unless a signed form is received and documented.

There is a two-month minimum on all classes. One month's notice from the first of the month is required to cancel any class. Withdrawals must be done by filing and signing the Add/Drop form. Withdrawals must be done with the front desk and not by informing the teacher. Withdrawals must occur within the first 7 days of the month to be effective for the next month. No withdrawals will be accepted after January 1, as the student will be financially responsible for the remainder of the year in all classes enrolled in.

Inclement Weather


All cancellations will be posted on the studio website and communication app. Because of the many different counties represented in the studio, please refer to the message for all cancellations

Attendance/ Make up policy
Dance requires discipline and consistency. For this reason, we expect students to attend class every week. When a student misses a class, it not only hinders their performance, it hinders the overall progress and cohesiveness of the class. No refunds/credits are given for Dancers missing class, classes cancelled due to inclement weather, missed classes due to vacation, Holidays and/or days off. Make-up classes are available for absences due to illness or other personal reasons, during regularly scheduled classes; provided however, missed classes must be made-up within one (1) month of the absence and can only be made up in a class of the same dance discipline, of equal or lower level. Registration at the front desk for make-up classes is required. Missed classes may not be applied to unpaid or overdue tuition. Days are typically set aside to make up for classes cancelled due to inclement weather. Classes with less than 4 students registered or in attendance, are subject to cancellation or the class may be merged with another class. On days where only 1 or 2 students show up for class, class will only be 30 minutes in length. There is no prorate for these, or any classes.
