I realize that participation in dance classes and related activities could result in some possible personal injury. Despite precautions being taken by the studio, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by The Dance Compound Inc.
LIABILITY & PROPERTY - I/we agree to release from responsibility the The Dance Compound Inc., located in Calgary, Alberta, including all teachers, dancers, and staff members of The Dance Compound Inc., from any cause of action, claims, or demands now and in the future. I/we will not hold The Dance Compound Inc., liable for any personal injuries sustained or illness contracted while attending or participating in any dance classes, rehearsals, workshops or performances, and including, but not limited to,: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death, or any personal property damage/loss, which may occur on the premises before, during or after classes, rehearsals, workshop, or performances.
Furthermore, I/we agree to obey the rules set forth by The Dance Compound Inc and all facilities, and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by The Dance Compound Inc.
I understand that The Dance Compound Inc. is an insured organization. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Studio Owner, Faculty, or staff members as soon as possible.
MEDICAL EMERGENCY - In case of physical injury or medical emergency, I hereby authorize The Dance Compound Inc. to make necessary arrangements to transport myself or my child to a medical treatment facility, as necessary. All such transportation and medical treatment will be at my sole cost and expense. In an extreme emergency, or if my child is under 18 years of age, I understand that The Dance Compound Inc. will attempt to notify the person(s) I have named below as my emergency contact(s).
