1. Terms and Conditions:
I have read, understood, and agree to abide by all the Terms and Conditions of Enrolment provided by Perth Circus & Dance Academy (PCDA).
2. Membership and Payment:
I understand that enrolling in PCDA constitutes a membership, and tuition fees are paid via weekly direct debit. I acknowledge that payments will continue until I formally withdraw my enrolment.
3. Withdrawal Policy:
I agree to provide a minimum of two (2) weeks’ written notice if I wish to withdraw my enrolment from any class. Tuition fees will continue to be charged during this notice period, regardless of attendance.
4. Missed Classes and Refunds:
I understand that missed classes are non-refundable and that make-up classes are not offered.
5. Costume and Performance Fees:
I acknowledge that additional fees, including performance fees, may apply during the year, and I agree to these charges as outlined in the Academy’s policies.
6. Photography and Media Consent:
I give permission for PCDA to use photographs or videos of my child for promotional purposes, including social media and marketing materials.
7. Code of Conduct:
I agree to ensure that my child follows the Academy’s Code of Conduct, maintaining a positive and respectful environment for all students, staff, and families.
8. Liability Waiver:
I understand that while all reasonable care will be taken, participation in dance and circus activities involves some risk. I agree that PCDA will not be held responsible for any injury or loss incurred while attending classes, performances, or related activities.
