Waiver of Liability
(Motion Studio, LLC) I am the legal guardian of the aforementioned student. I give full permission for participation in all classes whether on the premises of Motion Studio, LLC or via the internet, programs and performances associated with Motion Studio, LLC. I accept all risks associated with that participation and understand that there is a full possibility of serious physical injury or illness. I hereby covenant not to sue and waive, release and forever discharge any and all rights and claims for damages, which may arise now or in the future against Motion Studio, LLC and its owner, employees, subcontractors, and/or other assigned representatives or volunteers from any and all liability and for any and all damages and/or injuries which may be sustained or suffered by the student(s) listed while participating at or for Motion Studio, LLC.
Permission for treatment- I authorize Motion Studio, LLC, its owner, employees, subcontractors, etc. to use standard first aid procedures on the student(s) listed and to consent any other medical procedures that are deemed necessary in the case of an emergency. Furthermore, I certify that I personally and/or my medical insurance carrier will be responsible for all expenses which are incurred in relation to any injury sustained during any Motion Studio, LLC related activity, including but not limited to Motion Studio, LLC classes, performances, etc.
Conduct
Students are expected to observe Motion Studio's dress code, and standard of conduct and we reserve the right to discontinue classes for students that do not. Families are expected to read the Motion Studio Handbook and observe the standards and policies set forth therein. Family members in waiting areas must show respect for classes by being quiet and keeping children not in class under supervision at all times.
Tuition and Payments for Recurring Classes
Simple and consistent tuition payments for your budget. We keep your life simple and easy with a convenient 12-month payment system with tuition for group classes averaged out into automatic payments on the 1st of each month. For all group classes, your tuition will be the same July- June regardless of the number of classes that fall in the month. For all private lessons, your tuition will depend on exactly how many lessons will be received in the coming month. (Classes that do not participate in recital will end at the end of May and will not pay June tuition.) A non-refundable recurring yearly membership fee ($55 per student/ $70 per family) will be due upon enrollment and will be charged to your account automatically each year on July 1.
Photography Release
I hereby release all photography, still or motion footage, and/or any recordings of the voice of the listed student(s) for advertising, publicity, etc. for Motion Studio, LLC.
If you DO NOT want your child's image to be used in Motion Studio marketing, please contact the front desk so we can make a note on your account.
Communication
Newsletters, updates, account statements, etc. will be emailed. Parents are expected to read emails from Motion Studio.
