As parent/guardian/18+ student, I agree to the following:

I certify the enrolled dancer is in good health, physically fit and may participate in activities at Rhapsody Studios and The Sydney Ballet Conservatory. All participation in dancing lessons, competitions and events for and at Rhapsody Studios is at my own risk. I understand that it is my responsibility as a parent/guardian/student to inform Rhapsody Studios if my child or self is unfit to participate in any activity.

In case of an emergency requiring urgent medical treatment, I hereby authorise Rhapsody Studios to have the student taken to a medical or hospital facility for treatment.

I understand:
- All families must be signed up for DanceStudio-Pro Auto-Pay. Bank details or card details must be entered upon enrolment, or my child cannot attend class until completed.
- All fees are paid via monthly instalment from my nominated card or bank account.
- Bank accounts will incur a 1% surcharge and credit cards will incur a 2% surcharge.
- A dishonour fee of $10 will apply to any debits that are not successful on their first attempt.
- If there are no payments after 2 weeks, in the 3rd week, my child will not be able to participate in class.
- There is a 1-month cancelation notice period for canceling out of some or all classes. Once Rhapsody Studios is notified in writing of a class cancellation, I will remain enrolled into the class (and can continue to attend) until the next month’s instalment is successfully deducted, then the change will be actioned.
- My monthly fees and base costs (concert, registration, admin, etc) are charged monthly and are non-refundable.
- None of our inclusions are refundable. If you do not take advantage of our inclusions, then they are forfeited.
- That students with outstanding fees will not be permitted to perform in the end of year concert.
- Additional rehearsals may be called throughout the year based on concerts and other performances.
- All newsletters, notices and information packs are forwarded via email and that all information held within is assumed read.
- Our enrolment includes a guaranteed 36 weeks of classes. Any additional classes provided beyond this period are considered complimentary.
- Rhapsody Studios and The Sydney Ballet Conservatory are not financially liable for classes or dates cancelled due to events beyond our reasonable control, such as extreme weather conditions, competitions, performances or ballet exams.


I agree that Rhapsody Studios or anyone employed or volunteering for Rhapsody Studios will not be held responsible for personal injury or loss/damage of property to me, or my child:
- When I am/they are on the premise of Rhapsody Studios
- When I am/they are involved in activities external to Rhapsody Studios, but organised, approved or endorsed by Rhapsody Studios
- When people external to Rhapsody Studios are using the studio space
- When people external to Rhapsody Studios are involved in other events (lessons, competitions, events)
