I acknowledge that the contagious nature of COVID-19, COVID-19 VARIANTS, and other contagious diseases and viruses and voluntary assume the risk that I am/or my children may be exposed to or infected by COVID-19 or it's variants by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and variants and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including but not limited to, employees, independent contractors, volunteers, and program participants and their families. I release Theory Dance LLC and The Spot, its instructors, independent contractors, and all associates from liability for harm, injury, or death pertaining to COVID-19 and other contagious diseases and viruses.

I , the undersigned parent/guardian, do hereby give my permission for my child to attend and participate in dance classes and performances with Theory Dance, LLC's instructors or staff. I authorize Theory Dance, LLC and/or The Spot instructors or staff to administer first aid and to obtain medical care for my child in the event of an accident or injury. This emergency consent form verifies that the above-named participant is in good health and has her/his doctor's approval to participate in dance classes in person and virtually. I acknowledge that Theory Dance, LLC and The Spot has no medical insurance coverage and will not be responsible for injuries. In the event of an accident or illness involving the participant, the parent/guardian will be notified immediately. In the event of an emergency and I cannot be contacted, I hereby authorize Theory Dance, LLC and The Spot to take any steps they deem necessary to obtain medical attention. I agree to be responsible for all costs incurred as a result of the foregoing. This authorization is given pursuant to the provisions of Section 25.8 of the California Civil Code.

Tuition is to be paid by Automatic withdrawal (Auto-Pay) by the 1st of the month or a $25 late fee will be added. If you do not want your card to be drawn via Automatic withdrawal, then you must pay before the 1st of the month. If a payment is 30 days overdue then your child will be dropped from the class. If a payment is not received within a period of 4 months then the account must be handed over to a collection agency. Theory Dance, LLC does not prorate tuition for personal absences (i.e.vacation or illness)

I hereby grant and authorize Theory Dance, LLC the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of my child to be used and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, websites, fundraising, journalists, social networking sites and other print and digital communications, without payment or any other consideration. We use photos and videos on our Instagram, Facebook, and Website. This language extends to all language media formats and, markets now known or hereafter devised. This authorization should continue indefinitely, unless otherwise revoked in writing. I understand and agree that materials shall become property of Theory Dance, LLC and will not be returned. I hereby hold harmless, and release Theory Dance, LLC from all liability, petitions, and causes of action, which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my child's behalf or on behalf of my estate. I warrant my child's consent and am competent to contract in my own name, I have read this release before signing and I fully understand the contents, meaning and impact of this release.
