I understand that Evolution Dance Company-MN does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Evolution Dance Company-MN, and I voluntarily assume the risks associated with such participation. I understand that if I were to drop a class, a 30 days written notice is needed in order to stop tuition. I further understand that once apparel, costumes and competition fees are paid for, no refunds will be given. If you drop a class no refunds will be given for the months your dancer was in class. If weather or state mandates cause Evolution Dance Company to close, no refunds will be given.
Evolution Dance Company uses photographs and videos of students for publicity, marketing, website and social media platforms. I grant Evolution Dance Company permission to use photos and videos of my child for these purposes.
