PAYMENT POLICY: All Payments are due on the 25th of the month prior to the charge month. EX: February tuition is due January 25th. If payment in full is not received BEFORE the 1st of the month a $15 per family late fee will be applied. Email bills are only a reminder. If emails are not received payment is still due on time. Automatic Payments will be charged on the 25th of the month prior to the charge month. Ex: February 25 will be the charge date for March Tuition. If your card cannot be charged and you have not paid the balance before the 1st of the month, a $15 late fee will be applied. If the card is unable to be charged 2 months in a row, the card will be deleted from the system and the family will no longer be enrolled in auto pay. If a prepaid last month tuition was waived at sign up, the fee will be charged to the account and must be paid before attendance in any future classes. If you do not sign up for automatic payments you must pay your account on the 25th each month. Late fees are applied to all unpaid accounts on the 1st of the month. We accept cash, checks (to J31 Dance Center), and Credit/Debit Cards. If you pay with a card we do not store your card information unless you fill out paperwork to enroll in automatic payments. If your account is unpaid for 30 days your child(ren) will be dropped from the class. There is a $20 fee for all returned checks and NSF transactions., Prices and fees are subject to change at any time. REGISTRATION FEE: Students will be charged a registration fee upon enrollment per session. Families will be charges a maximum of $60 per session.
MAKEUP POLICY: Because of our strict student to teacher ratio, missed classes will not result in make-up classes, prorated tuition or refunds. No refunds for classes missed including scheduled holidays. Makeup classes will be scheduled by J31 Dance Center in the event of multiple closings.
DROP PROCEDURE: PARENTS MUST NOTIFY THE STUDIO IN WRITING TO DROP A STUDENT FROM CLASS by completing the Drop form. Only the official Drop form will be excepted. The parent turning in the drop notice must be the primary billing contact listed on the account. J31 must receive notice on or before the 20th of the month to drop at the end of the month. EX: February 20th is the deadline to drop for February. Notice received after the 20th of the month will be applied to the following month. EX: February 21st drop notices will be applied to March. Please note: You are responsible for payment for the spot held for your student in class WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time you notify the staff VIA DROP FORM. Please do not rely on your student to verbally let us know that he/she will no longer be attending classes. If a student stops coming to class without notification then that student's account will be charged for the additional 60 days. This charge will be for holding the student's place in that class instead of offering that place to one of the many on a waiting list. All initial fees: Anniversary, First Month Tuition, and Last Month Tuition are non-refundable and non-transferable.
COMMUNICATION: We use numerous forms of communication to reach our families. It is extremely important that you have an email address on file and check it regularly. We also have our very own J31 App that you can download from your app store. Please see the QR code at the front desk. This is an important tool for push notifications such as closing for weather. We also utilize social media so make sure and like and follow all our pages! You are always welcome to contact the studio and leave a message if you have a question. Studio Hours: MON-THUR 4:30 - 8:30pm.
PHOTO RELEASE: I give permission for J31 DANCE CENTER LLC to photograph my child during class and reproduce the photographs taken for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium. J31 will ask your permission before using photos for advertising. All team photos and photos of classes and special events will be published on our Facebook page and website.
WHAT TO WEAR: Leotard and tights are required for all classes. Black leotard and pink tights are required for ballet/pointe. NO loose fitting clothing. NO chewing gum or dangling jewelry. Hair should be pulled neatly and securely away from the face so that is stays up. Girls should not wear bows or other large hair ornaments that may cause discomfort during activity. All students should have appropriate footwear during class (tap shoes, ballet shoes, etc). Personal items should be left in cubbies. Jewelry should not be worn during classes. PLEASE LEAVE JEWELRY ARTICLES AT HOME. This facility's staff will not be responsible for ANY items that may be lost or stolen. Be sure your student's personal items are marked with their name. If dancers are not dressed appropriately for class they will be asked to leave class until they are prepared. ARRIVAL AND PICKUP: Be sure your student arrives 5 minutes before (no earlier please) his/her scheduled class time. Please pick up your student on time. Please inform us if you know you will be late picking up your student. Instruct your student to wait inside the building and you should escort them from the building to your car. During peak times the parking lot is crowded. Please take into consideration that our students include young children. Please drive slowly and carefully. Do not take a chance on your student running to and from your car.
WHERE TO WATCH: We have televisions in the lobby with a live feed to classes. Please refrain from communicating with your child during class, as this is both a distraction and a safety hazard. Please help keep our seating area neat and clean for our next class. Parents are not allowed to enter the studios with dancers. Teachers will help students get shoes changed and prepared for class.
BAD WEATHER: We rarely close due to weather. If we do close we will notify you via the email address you provide on file. If you have opted out of our email blasts you will not receive cancellation emails. Cancellations will be announced on our website and facebook page.
J31 DANCE CENTER, LLC PARENTAL PERMISSION & ACKNOWLEDGEMENT OF RISK, RELEASE & WAIVER OF LIABILITY, PARENTAL INDEMNITY, AND MULTIMEDIA LICENSE AGREEMENT.
I. ACKNOWLEDGMENT OF RISK & PARENTAL PERMISSION: I have been informed and understand that the dance activities sponsored by J31 Dance Center possess inherent risks, including: RISKS ASSOCIATED WITH DANCE/CHEER AND TRAINING, AND POSSIBILITY OF SERIOUS BODILY INJURY. I acknowledge and assume these risks and hereby give my approval and consent for Participant(s) to participate in J31 Dance Center activities. I have no knowledge of any reason that Participant(s) is(are) not fully physically able to participate in J31 Dance Center activities, and I agree to inform J31 DANCE CENTER immediately if I learn at any time in the future that Participant(s)_ is(are) not fully physically able, whether from injury, illness, or otherwise, to participate in J31 DANCE CENTER activities.
II. RELEASE & WAIVER OF LIABILITY: In consideration of Participant(s) being allowed to participate in any classes, performance team, or Company team, or any other activity sponsored by J31 DANCE CENTER and the execution of this Agreement, I EXPRESSLY AGREE THAT J31 DANCE CENTER SHALL NOT BE LIABLE FOR ANY DDAMAGES ARISING FROM PERSONAL INJURIES SUSTAINED BY PARTICIPANT(S) OR MYSELF WHILE PARTICIPATING IN ANY OF J31 DANCE CENTER'ACTIVITIES, AS A RESULT OF ANY ACTIVITIES, WHILE USING ANY OF THE J31 DANCE CENTER FACILITIES, OR WHILE PRESENT ON THE J31 DANCE CENTER PREMISES. i ASSUME FULL RESPONSIBILITY FOR ANY SUCH INJURIES OR DAMAGES WHICH MAY OCCUE TO THE PARTICIPANT(S) OR MYSELF IN, ON, OR ABOUT THE J31 DANCE CENTER PREMISES OR WHILE USING THE COMPANIES FACILITIES OR WHILE PARTICIPATING IN OR AS A RESULT OF THE PARTICIPATION IN THE COMPANIES ACTIVITIES. I FURTHER AGREE THAT J31 DANCE CENTER SHALL NOT BE LIABLE FOR ANY LOSS OR THEFT OF PERSONAL PROPERTY. I ALSO SPECIFICALLY AGREE THAT J31 DANCE CENTER SHALL NOT BE RESPONSIBLE FOR SUCH PERSONAL INJURIES, DAMAGES, LOSS, OR THEFT IN THE EVENT OF NEGLIGENCE BY THE J31 DANCE CENTER, WHETHER SUCH NEGLIGENCVE IS PRESENT AT THE TIME OF THE SIGNING OF THIS AGREEMENT OR TAKES PLACE IN THE FUTURE. THIS RELEASE AND WAIVER DOES NOT APPLY TO GROSS NEGLIGENCER, INTENTIONAL TORTS, OR ACTIVITIES AGAINST THE PUBLIC INTEREST BY J31 DANCE CENTER.
III. PARENTAL INDEMNITY: If I, Participant(s) or anyone on Participant(s) behalf, make a claim based in negligence, including future negligence, against J31 DANCE CENTER, I agree to indemnify and hold harmless J31 DANCE CENTER from any loss, liability, or costs, including legal fees and costs and expenses, arising from such claims.
IV. MULTIMEDIA LICENSE AND RELEASE: I hereby grant to J31 DANCECENTER permission to photograph, video, or record images and sounds (hereafter REFERRED TO AS "Multimedia) of Participant(s) during any of J31 DANCE CENTER a perpetual royalty-free exclusive license to reproduce in any manner or medium such Multimedia for the purpose of publication, promotion, illustration, advertising, or marketing.
V. SEVERABILITY: In case any one or more of the provisions contained in this Agreement shall for any reason be held to be invalid, illegal, or unenforceable in any respect, such invalidity, illegality or unenforceability shall not affect any other provision hereof or thereof, but each shall be construed as if such invalid, illegal or unenforceable provision had never been included.
I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THE POLICIES AND GUIDELINES THAT HAVE BEEN DISCUSSED WITH ME AND PROVIDED IN WRITING BY J31 DANCE CENTER. I UNDERSTAND THAT IT IS MY RESPONSIBILITY TO BE SURE MY DANCER/CHEERLEADER COMPLIES.
