I understand and agree that in participating in any dance class, workshop, rehearsal or
performance, there is a degree of risk of strain or bodily injury. I voluntarily agree, therefore, to
assume all risks and responsibility for any such injury or accident, which might occur to me or
my child during any of Dance Moves Maine classes, rehearsals, performances, or activities. I
understand that these activities may include, without limitation, dancing with props, stretching,
barre work, dance routines and other related activities and that my child will be one of several
under the supervision of agents or employees of Dance Moves Maine.
I hereby release Dance Moves Maine, its owners, agents and employees, including volunteers,
assistants, guest artists, and/or students, from any and all liability for bodily or personal injury,
and for the loss of damage to personal property, which may occur during or arising out of my/his/her
participation in the program. I further agree to waive any claims or cause of action that I may
have, and to indemnify and hold Dance Moves Maine, its owners, agents, and employees,
including volunteers, assistants, guest artists and/or students harmless from and against any
and all claims, demands, causes of action, losses, damages or expenses related in any way,
directly or indirectly, to my child’s participation in the program.
I understand that I should be aware of my physical limitations and agree not to exceed them. If I
am signing this waiver for my children, I certify that I am the parent or legal guardian and have
the right to waive these rights.
I hereby acknowledge that I have read this document carefully and that I fully understand the
contents.
