By signing below you are agreeing in full to the following; 1. When enrolling as an AAPA student you are enrolling as a student who dances at/and only AAPA. 2. You agree to pay invoices by the due date or arrange a payment schedule. Failure to pay your invoice within 7 days of the due date will incur a $15 late payment fee. If your invoice remains outstanding for 14 + days your child may be asked to sit out on class until such time as payment &/or an arrangement for payment is made. 3. You are agreeing that you have been made aware fees are non-refundable. 4. Timetables are subject to change throughout the year. Notice will be given. 5. You give permission for AAPA to use video footage and / or photographs of your child. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to: printed materials, online and offline advertising and promotion, videos and digital images such for use on Social Media. By signing this form, you acknowledge that you are giving unrestricted permission for your child’s image to be used in print, video, and digital media. I agree that these images may be used by AAPA for a variety of purposes and that these images may be used without further notification. 6. All choreography and artistic creations by AAPA are property of AAPA and are not to be used without permission. 7. AAPA is not responsible for students before or after classes. Please make sure students are respectful of starting times of classes and that all students are picked up on time. 8. You agree to show respect to all students and teachers and acknowledge that AAPA is a bullying free zone. 9. You agree to purchase and have your child/children wearing the correct AAPA uniform applicable to your chosen style. 10. You acknowledge that participation in dance is potentially dangerous and there is an inherent risk of injury involved. In allowing your child to participate in AAPA activities, You hereby assume all the risks associated with the performing arts. You understand the importance of you and your child following the instructions and rules set by their instructor/s, and you agree to release AAPA and it’s employees of any and all liability which may arise as a result of your child’s participation in activities at AAPA. 11. You acknowledge that in the unlikely event of an injury to your child AAPA will call an ambulance if deemed necessary which will be at your expense. 12. You understand and respect that teaching techniques will vary between classes but will involve physical teacher student contact at times as part of our safe dance practise and when giving anatomical corrections. 13. You understand that AAPA uses a drop off system and that you will not be able to watch any classes.
