I understand that the monthly dance fees are calculated into EQUALIZED PAYMENTS. Families will receive 35 weeks of dance lessons, with payments spread out over 10 monthly payments from August 22 - May 22. The registration fees & half tuition paid at the time of registration are non-refundable if I should cancel at any point during the year. Any tuition already paid during the year is also non refundable if you should cancel. I must pay any outstanding fees from the current season before my registration will be completed. You will be notified of any fees owing by email. All notifications will include a deadline that the fees must be paid by. Any monthly dance fees overdue by 1 week will be subject to an additional late charge of 5%. All Classes are subject to change and/or cancellation. Excellent attendance is expected from all levels of students. I understand that the studio is not required to provide make up classes for any cancellations due to unforeseen causes (i.e., snow days, power outages, competition etc.) but will try to offer if time and space permits. Above student releases and discharges affinity dance inc. centers and teachers from any and all actions, causes of action, for damage, or loss of injury, arising that which may be sustained by me/child in consequence of my membership in the school. I also agree that affinity dance inc. centers shall not be held responsible for any loss or theft of the applicant's personal possessions.
I understand that my tuition will be charged to my credit card on Aug 22 - May 22. If the tuition payment does not go through and is overdue by 1 week it will be subject to an additional late charge of 5%. The DSP processing fees are 3.05% & .30 per transaction. In order to cancel/change my auto payments I will need to email Lauren at info@affinitydance.com to cancel from a class.

I hear-by consent to my child's participation in the taking of photos or videos on behalf of Affinity Dance Inc. I also grant the right to edit, use and reuse said products for print, online social media and all other forms of media, pertaining to Affinity Dance.
