By signing below, I also agree to the terms of my payment schedule as as well all studio policies. Failure to comply may result in dismissal from the program. Yearly tuition is broken into nine equal installments due monthly via recurring invoices from September-May.
RAVE Studios by Dance Progressions may videotape or take photographs of participants enrolled in a dance, exercise or performing arts program. RAVE Studios by Dance Progressions may videotape or take photographs of participants taking part in any sponsored special event or performance on or off site. These photos or videotapes may be used for promotional purposes.
Understanding that dance is a physical activity, I hereby waive and release RAVE Studios by Dance Progressions and its’ instructors from all rights and claims for damages caused by participating in such activity. I accept all responsibility for any condition that may arise due to participating in the dance or exercise programming at the RAVE Studios by Dance Progressions facilities and/or any off site RAVE Studios by Dance Progressions sponsored event or performance. I understand that there is an inherent risk of injury associated with dance related activity and I authorize emergency medical treatment and transportation to a medical facility in my absence.
