In consideration of participation at Elevate Dance Ministry/Elevate, I represent that I understand the nature of this Activity & that I am qualified, in good health, & in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that this Activity involves risks of bodily injury, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releases" named below; & that there may be other risks either known to me or not readily foreseeable at this time; & I fully accept & assume all such risks & all responsibility for losses, cost, & damages I incur as a result of my participation in the Activity.

I hereby release, discharge, & covenant not to sue Elevate Dance Ministry/Elevate, its respective administrators, directors, agents, officers, volunteers, & employees, other participants, & sponsors, advertisers, and if applicable, owners & lessors of premises on which the Activity takes place, (each considered on of the "releases" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releases" or otherwise, including negligent rescue operations & future agree that if, despite this realize, waiver of liability, & assumption of risk I, or anyone on my behalf, makes a claim against any of the releases, I will indemnify, save, & hold harmless each of the releases from any loss, liability, damage, or cost, which any may incur as the result of such claim.

I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights, & I have done so freely & without any inducement or assurance of any nature & intend it to be a complete balance, notwithstanding, shall continue in full force & effect.

I give permission for Elevate Dance Ministry owners, officers, employees, volunteers, and/ or agents to seek emergency medical treatment for the participant(s) in the event they are unable to reach any parent or guardian. The undersigned also agrees that they themselves will be responsible for any financial debt incurred by said action.

Occasionally Elevate Dance Ministry uses photos or video of its participants in print ads, on its websites, or other marketing mediums. I understand that Elevate Dance Ministry may use the likeness of any/all of the participants I have registered for advertising purposes. These images will be used for Elevate Dance Ministry purposes only & will not be given or sold to outside companies or individuals.
