I understand the following requirements for participation at Encore Performing Arts Studio, Inc.:

- A Registration Fee is processed upon placement in the selected class.

- Payments are automatically deducted on the first of each month, September - May, via the Dance Studio Pro portal unless otherwise specified.

- Additional costume fees will be assessed in January and February.

- Recital Fees will be assessed in March.

- If my account balance is not paid by the 5th of the month, late fees of $25.00 will be assessed.

- Withdrawal from class notices must be submitted in writing before October 31. If not received, the account and billing will remain active until May 2025.

I understand the credit card I provide will be auto-processed in the Dance Studio Pro portal for each tuition installment and specified fee dates. I understand Encore Performing Arts Studio Inc.'s payment policies and agree to make payments accordingly.

I understand all balances are due 30 days from the date of Invoice. If payment is not received within this time frame, the account is subject to interest on the unpaid balance at 1% per month (12% APR) on the indebtedness, in addition to costs associated with the collection process, including but not limited to court costs and reasonable attorney's fees.

I understand Encore Studio does not give credit and/or refunds for class(es) missed due to holidays, vacations, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Encore Studio. I voluntarily assume the risks associated with such participation.

Encore Studio is authorized to make, photograph, or record images of my dancer(s) and to incorporate and use such imagery in any photograph(s), video(s), and/or printed or recorded medium for any commercial, trade, or other lawful purposes as they shall desire and in any manner they deem advantageous including, for example, production and marketing materials to promote Encore Studio.
