Kohesion Dance Center Waiver/Policies

PAYMENT: {Only applies to Fall registration}
When registering, a one time $25 registration fee per family is due.
September-Junes tuition is due on the 1st of each month. Kohesion offers a 10 day grace period. If payment is not submitted by the tenth, a $20 late fee will be charged on the eleventh. If your payment is not submitted within 3 weeks, your dancer will not be able to attend class until payment is submitted.

We encourage auto-pay but it is not required! When using a card, we include a 3.5% processing fee.

COSTUMES: {Only applies to Fall registration}
Each dance class will have a costume for the recital with the exception of stretch & strength classes. Costumes cost $85 for child sizes and $95 for adult sizes. A deposit of $40 is due October 15th. The remaining balance is due December 15th. If costumes haven't been paid by December 20th, 2024, you will be considered late; at that time a $10 late fee will be charged per costume. Costumes are non refundable.

STUDIO COMMUNICATION: All communication at KDC is done through email. (kohesiondance@gmail.com) It is important to stay in the know about events, updates, billing, studio closings and announcements! Add us to your contact list to avoid messages going to spam.
The Studio opens for the 2024-2025 season for classes the week of 9/9 - 9/13 and closes on Saturday June 14th 2025 after our Recital for summer break.

FEES: All fees collected by KDC, including registration, tuition, costumes, apparel, footwear, accessories, props, etc., are entirely non-refundable, regardless of whether my child and/or I engage in any activities.

PERMISSION, SOCIAL MEDIA,INSURANCE AND RELEASE: As the parent/guardian of the dancer, I hereby grant permission for my child to partake in any activities organized by Kohesion Dance Center (KDC). I also authorize KDC to utilize photos and/or videos of students for various media platforms. Please note that KDC and its staff do not provide medical insurance for students or observers. It is mandatory for all individuals to have coverage under their own family insurance policies. In the event of injury or illness, it is understood that reimbursement will solely rely on the student's/observer's personal insurance policy. I, as the parent/guardian of the dancer, or as a student of KDC, agree to waive any claims and hold harmless KDC, its staff, participants, agents, or representatives—including those responsible for transporting the student/minor to and from KDC events—from any liabilities arising from injury or illness

RULES & REGULATIONS As the parent/guardian, I acknowledge that is my responsibility to ensure that my child is picked up promptly upon dismissal of his/her class, rehearsal, or any other scheduled event. By signing this Contract & Release form, I am agreeing to adhere to my obligations and payment terms until the last class of June 2025. I understand that tuition and late fees will continue to accrue until this form is completed and returned to the front desk. Additionally, I agree to comply with the rules and regulations of KDC, and I will communicate these expectations to my child. I understand that any non-compliance by either the parent or the student may result in disciplinary action, including possible removal from class, dance company, and/or studio.

Acknowledgment of Risks: I recognize and acknowledge that the activities in which I am about to engage may entail inherent risks and potential hazards. My participation is entirely voluntary, and I am fully aware of these risks, which may include, but are not limited to, physical injury, property damage, and loss.

Release and Waiver: I hereby release, discharge, and relinquish any and all claims, liabilities, or causes of action against Kohesion Dance LLC its officers, directors, employees, agents, and representatives (collectively, the "Releasees"). This release pertains to any loss, damage, injury, or even death that may occur to me or my property while participating in the activities.

Indemnification: I agree to indemnify and hold the Releasees harmless from any claims, actions, damages, costs, or expenses, including attorney's fees, arising from my participation in the activities or any breach of this agreement.

Medical Treatment: In the event of an injury or illness during my participation, I grant the Organizer the authority to seek medical treatment on my behalf, with expenses incurred at my own cost. I acknowledge that the Organizer bears no responsibility for any medical expenses.

Personal Belongings: I assume full responsibility for the safety and protection of my personal belongings during the activities. I release the Organizer from any liability regarding the loss, theft, or damage to my personal property.

Photographic Release: I provide the Organizer with permission to photograph and record dancers during the activities. I understand that these images may be utilized for promotional purposes and waive any rights or claims to compensation for their use.

Governing Law: This agreement will be governed by and interpreted under the laws of New Jersey without consideration for its conflict of laws principles.

Severability: If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain fully effective.

I hereby consent to participate in the activities organized and facilitated by Kohesion Dance Center.
As a condition of my participation, I acknowledge and accept the following terms and conditions:
I have read this liability waiver and release agreement, comprehending its contents entirely. I am aware that, by signing this agreement, I am voluntarily and freely giving up certain legal rights.
