Media: I acknowledge that I / my child will be videotaped, photographed for educational, performance, advertising, and website purposes. Names of subjects will not be published. I grant Boyd Dance Studios, its representatives, and employees the right to take photographs and/or videos of my child to use in the local paper, our website, social media, or advertisements. Payment Policy: I understand that monthly tuition is payable monthly in advance, due by the 1st of each month. Tuition remains the same whether it's a long (5 week) or short (3 weeks) month and regardless of absences, vacations, or holidays. No refunds or adjustments due to Absences. Tuition is non-refundable and non-transferable. Although, make-up classes are encouraged and must be taken within the Month missed. Tuition is considered late on the 1st of each month. I also understand that a $15.00 late fee will be assessed to my Account if not current on the 1st of each month, regardless of absences, illness, weather-related, vacations I also understand that in order to withdraw from Boyd Dance Studios, written notification is required before the 15th of the upcoming Month to avoid further billing. LIABILITY: In participating in a dance /or tumbling classes at "Boyd Dance Studios" (hereafter "The School") I am voluntarily participating with the knowledge of certain risks and dangers are involved, with a certain result of injury (such as strains, sprains or other injuries) as well as a possible risk of exposure to COVID-19. I hereby release, , hold harmless "The School", it's owners, member advisors, class, and all employees and agent parties from liabilities, whether on or off the premises to named student/ child while participating in this activity. I, on behalf of myself and the student/ participant named below do hereby voluntarily participate in any activities and all "The School" activities. I do agree that "The School" Shall not be liable.
