I, the undersigned, fully understand and agree to abide by the following:

My child will be participating in a physical activity, and as such, I am aware of the element of risk.
In consideration of myself, I, for myself, my heirs and assigns, hereby release Metuchen Dance Centre, the owner/directors June Vaganek, Alessandra Carannante, their family, and/or employees from any and all future rights, claims, causes of action, civil or criminal claims, demands, costs, attorney fees, loss of service, expenses, compensation, third party actions, suits of law or equity (including suits for contribution and indemnity), of whatever nature, and all consequential damages on account of, or in any way associated with Metuchen Dance Centre.

I also understand that by agreeing to these terms, there will be no refunds, pro-rations, or credits given for any reason.

Metuchen Dance Centre reserves the right to alter class schedules, for any reason, either temporarily or permanently, without fault or liability.

Should I decide to discontinue a class, Metuchen Dance Centre requires a written notice in order to assure proper credit on our tuition account. I will still be responsible for all tuition and fees until a written notice is attached to my child's file.
Tuition fees are non-refundable. There are NO refunds or adjustments for absences for any reason (illness, vacation, holidays, etc). Instead, I am welcome to take a designated make-up class. Students are permitted a maximum of 4 makeup classes per class for the duration of the season enrolled.

Any limitation in ability to participate due to a medical condition must be noted on this form as designated “Medical Limitation” below. Failure to note such a condition is a warranty by the parent that such condition does not exist.

In the event of an accident or illness involving my child, I will be notified immediately. In the event that I cannot be contacted, I hereby designate the person noted on this registration form to be contacted. In the event that neither I nor the person designated as emergency contact (on registration form) can be contacted, I hereby authorize Metuchen Dance Centre to take any steps deemed necessary to make medical attention available, including, but not limited to, physicians, hospital and/or any other medical services, and Metuchen Dance Centre shall have full discretion to make such decision. In such event, I further agree that any and all costs of such medical services shall be borne exclusively by me and Metuchen Dance Centre shall incur no liability.

All recordings, including but not limited to video, audio, still pictures, motion pictures of any event, including but not limited to recitals, rehearsals, dance acts, classes, demonstrations, and/or competitions remain the property of Metuchen Dance Centre with all rights of ownership thereby retained.

Metuchen Dance Centre reserves the right to drop any registrant from our registrar for:
Excessive lateness and/or absence;
Disruptive behavior;
Non-payment of tuition and/or other charges.

ARBITRATION – Any controversy or claim arising out of or relating to this contract or agreement dated below or the breach thereof, shall be settled by arbitration in accordance with the Commercial Arbitration Rules of the American Arbitration Association, and judgment on the award rendered by the arbitrator(s) may be entered in any court of law having jurisdiction hereof.
