I hereby acknowledge that the activities involved with team/company and class participation at Elevé Dance Center are inherently dangerous and involve the risk of serious injury.
I hereby release, indemnify, hold harmless and forever discharge ELEVÉ Dance Center and its agents, employees, officers, directors, affiliates, teachers, coaches, members, successors and assigns, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity, that I or CHILD ever had or may have, arising from or in any way related to CHILD'S participation in any of the events or activities conducted by, on the premises of, or for the benefit of, ELEVÉ Dance Center provided that this waiver of liability does not apply to any acts of gross negligence, or intentional, willful or wanton misconduct.

I understand that the activities that said CHILD will participate in are inherently dangerous and may cause serious or grievous injuries, including bodily injury from falling, breaking ankles, limbs, etc. damage to personal property and/or death. On behalf of myself, CHILD, my heirs, assigns and next of kin, I and said CHILD waive all claims for damages, injuries and death sustained to me or my property, that I or said CHILD may have against the aforementioned released party to such activity whether caused by the ordinary negligence of the released party or otherwise, with the exception of acts of gross negligence, or intentional, willful or wanton misconduct as indicated above.

CHILD has the necessary and requisite skills to participate in all facets of, and activities of and requested of this facility, except as noted below. The nature of the activities has been fully disclosed and any flyer, advertisement, or brochure relating to the participating activities is expressly made a part of this WAIVER AND RELEASE.

By this Waiver, I, on behalf of said CHILD, assume any risk, and take full responsibility and waive any claims of personal injury, death or damage to personal property associated with ELEVÉ Dance Center, including but not limited to training at the studio, using the studio and its equipment, participation in an/or instruction of classes, private coaching, choreography, rehearsals, recitals, conventions, competitions, events, and/or other dance, or tumbling related activities on and off the premises.

This WAIVER AND RELEASE contains the entire agreement between the parties, and supersedes any prior written or oral agreements between them concerning the subject matter of this WAIVER AND RELEASE. The provisions of this WAIVER AND RELEASE may be waived, altered, amended or repealed, in whole or in part, only upon the prior written consent of all parties.

The provision of this WAIVER AND RELEASE will continue in full force and effect even after the termination of the activities conducted by, on the premises of, or for the benefit of, ELEVÉ Dance Center whether by agreement, by operation of law, or otherwise.

I have read, understand and fully agree to the terms of this WAIVER AND RELEASE. I understand and confirm that by signing this WAIVER AND RELEASE said CHILD and I have given up considerable future legal rights. I have signed this Agreement freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated to me. My signature is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law.

All ELEVÉ Dancers and families…

-Will be friendly, kind, compassionate, and welcoming to the other dancers and guests at the studio and in their classes.
-Are expected to demonstrate respect for all instructors, assistants, staff members, and fellow students.
-Must embrace all students in your class, regardless of differences in skill level, age, or background.
-Must be supportive of one another as well as dancers of other studios. Dancers must encourage and praise each other’s hard work and accomplishments including positive comments, clapping for one another, and other similar types of recognition.
-Are expected to focus and limit distractions in order to pay attention to the dance teacher's instructions.
-Are expected to be punctual. It is imperative that all dancers participate in warm-up to ensure that their body is ready for physical activity. If a dancer misses warm-up, it could lead to an unnecessary injury.
-Must adhere to the dress code. This includes hair and attire. Dancers that do not adhere to the dress code may be asked not to participate in class that day.
-Must treat the studio, the premises, and the possessions of all other dancers with respect, care, and consideration.
-Are expected to clean up after themselves and any mess that they create while at the studio.
-Are expected to be helpful. If you see a way you can help a teacher, a parent, or another student, do it!
-There is no tolerance for bullying of any kind, whether it be in person, on social media, by phone, email, or texting. Bullying will result in expulsion from the studio without refund.

I hereby consent and agree that ELEVÉ Dance Center has the right to take photographs, video tapes, or digital recordings of my child herein after referred to as "materials" and use them in prints and/or on social media such as a website, facebook, or instagram account.
I hereby release you, your representative, employees, managers, instructors, and all other staff members from all claims and demands arising from the use of said "materials".

ELEVE Dance Center Studio Policies

Auto-pay: All dancers must be enrolled in auto-pay in order to register. Tuition is posted on all accounts on the 25th of the month and due on the 1st of the month. I agree to pay all tuition and fees each month. If tuition is late, there will be a $20 fee. If the card on file declines, your account will be charged $25. Tuition is based on the number of classes for the full year and divided evenly through-out the 10 months, because of this months that are shortened or have weeks off due to school breaks are still the same price. We offer a sibling discount of $5 off tuition for the sibling(s). Tuition and fees paid via credit/debit card will have 2.9 + .30 cents added to each transaction. Tuition and fees paid via E-Check will only have a 1% fee. Tuition can also be paid via Venmo @elevedance or in cash at the studio. Tuition is a per hour charge. Amounts can be found on our website, elevedancecenter.org

Fees: All registration and recital fees are NON refundable. No exceptions. For summer classes, there aren’t any additional fees. For Fall/Winter classes, there is a yearly $35 registration fee charged at signup. There is a $50 costume fee due September 10 and October 10. If your dancer signs up after those dates, we will charge a $100 costume fee to try to order the costume in a timely fashion. In lieu of having families buy recital tickets, we charge a $50 recital and music licensing fee that’s due November 10 and February 10. If your dancer signs up after the Winter Recital, the first charge will be waived.

Cancellation Policy: We understand that life happens, kids decide to try other hobbies, and sometimes it just doesn’t work to stay in dance. For this reason, we do allow drops through-out the year with no additional fee (for recreation classes). We do however require a drop form to be filled out before the 15th of the previous month, any drop forms filled out after the 15th will be pushed to the next month in order for us to fill your child's spot in the class. (If you would like to cancel for February, you would need to fill out a drop form before the 15th of January.) No refunds will be given for classes unattended or any registration fees, costume fees, competition fees or recital fees paid. To get a drop form, please email us and we will respond within 24 hours.

Attendance: If your child cannot attend class due to illness or any other conflict, make-up classes are available. For example, if your child is absent for their regular ballet/jazz class they are welcome to attend an extra dance class appropriate for their age. Please email us and we can get that set up for you. Because your child's teacher will be present and performing her job whether or not your child is in attendance, there are no refunds or pro-rating available for classes missed. You are responsible for tracking your own make-up class attendance and there is no time limit for completing make-up classes. For instance, if your child misses in September but can't make it up until December, that is fine. We are flexible.
