Dance Studio Liability Waiver

I, ______Student and/or Parent/Guardian______, hereby acknowledge that I am voluntarily participating in dance classes, workshops, and activities offered by Impact Dance Academy LLC. I understand that dance involves physical exertion and poses inherent risks of injury. In consideration of being allowed to participate in these dance activities, I agree to the following terms and conditions:

1. Assumption of Risk: I am aware that dance activities may involve physical contact, stretching, and repetitive movements, which may result in injury. I understand and voluntarily assume all risks associated with participating in dance classes, including but not limited to sprains, strains, falls, and other potential injuries.

2. Physical Fitness: I affirm that I am in good physical condition and have no medical conditions or injuries that could prevent me from safely participating in dance activities. I understand that it is my responsibility to consult with a healthcare professional before starting any dance program if I have any concerns about my physical condition.

3. Release of Liability: I hereby release and hold harmless Impact Dance Academy LLC, its instructors, employees, and representatives from any and all claims, liabilities, damages, injuries, or losses arising from or in connection with my participation in dance classes or related activities at the studio.

4. Consent for Emergency Medical Treatment: In the event of an injury or medical emergency during my participation in dance activities at Impact Dance Academy LLC, I authorize the studio's staff to obtain necessary medical treatment for me and agree to be financially responsible for any medical expenses incurred.

5. Photographic Release: I grant Impact Dance Academy LLC permission to use photographs or videos of me taken during dance classes or events for promotional and marketing purposes, without any compensation to me.

6. Compliance with Rules and Instructions: I agree to abide by all studio rules and follow the instructions of the instructors during dance classes and activities.

By signing this waiver, I acknowledge that I have read, understood, and agreed to the terms and conditions outlined above. I understand the risks involved in participating in dance activities and voluntarily assume these risks. I hereby waive and release any claims against Impact Dance Academy LLC and its representatives.
