I hereby grant permission for my son/daughter/myself to attend dance classes and special events.
My child /I has/have no medical condition other than stated above that would interfere with
his/her/my participation. I release Ballet Folklorico Sol de San Antonio and the staff from all
liability or from any injuries that may occur during classes or special events. I understand and
grant permission of the use of all photos and videos to be posted on website or social media for
the sole purpose of marketing, studio announcements and promo videos.

Tuition is due on the 1st of the month. First month’s tuition is due at time of registration along
with the annual $30.00 registration fee per student ($15 for each additional family member).
Registration fees are non-refundable and non-transferable. Tuition is also non-refundable and
non-transferable once a tuition period has started. It is the responsibility of the Parent/Guardian
to read though and agree to ALL Studio & Tuition Policies, which will be provided at
registration. Your Signature on this registration form constitutes an agreement with Ballet
Folklorico Sol de San Antonio and demonstrate that:

San Antonio (BFSDSA) Event Organizers, Directors its officers, members, employees and volunteers
from any and all claims of participant arising from participation in any event and/or class put together by
BFSDSA. In exchange for the right to participate in the event or class, participant hereby waives,
releases and discharges any and all claims for injuries or sudden illness to person including death or to
property that participant may incur as the result of participants' participation in the event or class. This
release discharges BFSDSA Event Organizers, Directors its officers, members, employees and
volunteers associated with the event or classes, and from and against any and all liability which may
arise out of the negligence, carelessness, gross negligence or intentional act on the part of the BFSDSA
members, employees and volunteers.
PARTICIPANT understands and acknowledge that a serious accident can occur during activities such
the event or classes, knowing such risks PARTICIPANT hereby assumes all those risks and injury to
person including death or to property related to event and releases and holds harmless and indemnifies,
the BFSDSA employees, organizers, volunteers, staff and others involved in the organizing of such
event or class for any such injury whether such injury arises from the BFSDSA or others negligence,
carelessness, gross negligence or intentional acts, including but not limited to all costs and attorney's
fees.
Further, PARTICIPANTS hereby grant full permission to any or all the foregoing to use any
photographs, video tapes, motion pictures, recordings, or any other record of these events.
PARTICIPANTS has carefully read and understands the foregoing waiver and release and sign this
release as PARTICIPANT'S own free act. PARTICIPANTS represents in executing this document, that
the party doing so has the full authority to enter into this agreement with BFSDSA, Event organizers,
staff, volunteers, employees all others involved and acknowledges that BFSDSA and others involved, is
relying upon the execution of this document in allowing PARTICIPANT to participate in the event or
class.

We ask for your 100% commitment to our organization. Therefore, no dancing with other groups unless
it is with a High School or College. But you are still required to attend rehearsals or performances.
Parent and student participation is very important in any group activity, event, fundraisers, ticket sales,
group private or public performances, yearly recital and group production.
Punctuality is necessary. Arrive at least 10 minutes before class and start stretching.
Be courteous to everyone.
Absolutely, no gossiping, ear whispering will be tolerated.
Full participation is required during all class and rehearsals.,
No phones allowed on dance floor and all phones shall be placed on silence at all times before class.
Be ready to give 110% at all times. Do not be afraid to challenge yourself. We will not be afraid to
challenge you!
Always make the director aware of all problems, conflicts and/or concerns you might have. DO NOT
confront or approach individuals you might have concerns with. Talk to director and the problem will
be addressed in a professional manner.
No conversations in group chats, if you have a question please notify director directly.
Accounts must be current and have a $0.00 balance before the student can participate in
performances and receive costumes. Students taking private lessons must also maintain $0.00
balance.
All payments made to the BFSDSA are non-refundable. There are no deductions for classes missed.
This is a yearlong commitment/membership so agreement to pay monthly tuition even if student is
absent for any reason is understood and agreed upon.
