In consideration of being permitted to participate in dance classes and activities at Inspire Dance Complex LLC (the "Studio"), I, (or Parent/Guardian if under 18), hereby acknowledge and agree to the following:
Assumption of Risk: I understand that participation in dance activities involves inherent risks of physical injury, including but not limited to:
Strains, sprains, fractures, and other musculoskeletal injuries
Falls, collisions, and other accidental injuries
Illness or medical conditions exacerbated by physical activity
I voluntarily assume all risks associated with participation in dance classes and activities at the Studio, and I release the Studio from any liability for such injuries or conditions.
Release of Liability: I hereby release and forever discharge the Studio, its owners, instructors, employees, agents, and representatives (collectively, the "Released Parties") from any and all claims, demands, damages, actions, causes of action, or suits of any kind or nature whatsoever, whether known or unknown, arising out of or in any way connected with my participation in dance classes and activities at the Studio.
Medical Authorization: I authorize the Studio to provide emergency medical treatment, if necessary, and to transport me to a medical facility for treatment. I agree to be financially responsible for any medical expenses incurred as a result of such treatment.
Photo/Video Release: I grant the Studio permission to use my (or my child's) photograph, video, or likeness in any promotional materials, including but not limited to social media, website, and print publications.
Binding Agreement: This waiver and release are binding upon me, my heirs, executors, administrators, successors, and assigns.
By checking this box and registering for dance classes, I acknowledge that I have read and understand this waiver and release of liability, and I agree to its terms and conditions.
