I agree to release and hold harmless Slava Doval’s DanceFusion ( SD DanceFusion), including its teachers, subcontractors and facilities used by SD DanceFusion from any cause of action, claims, or demands now, and in the future. I will not hold Slava Doval or DanceFusion liable for any injury or any personal property damage, which may occur on the premises before, during or after classes.

Furthermore, I agree to make sure I, or my child(ren), respect the class and studio rules and take full responsibility for my, or my child(ren) behaviour in addition to any damage I, or they, may cause to the facilities utilized by SD DanceFusion.

I/we realize that participation in dance classes and related activities could result in some possible personal injury, or illness. Despite precautions being taken by the studio, accidents, injuries, and/or sickness may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to attending SD Dance Fusion classes and the spaces where the classes occur.

I give permission for my, or my child(ren)’s photo/video to be taken for use by SD DanceFusion.
If there is a safety issue with this I will email Slava.

I/we understand that we must arrive at class at the assigned time, and if unable to attend class will email to notify the staff of the absence.

I understand there are no refunds and no credit for missed classes for any reason.

I understand that if an instructor is unwell and the studio can't get a substitute teacher (we will do our best!) the class may be cancelled and there may be no make-up class or credit.

I understand that if there is severe weather/power outage/ or other unexpected circumstances and the class needs to be cancelled, there may be no refund/credit.

If my/my dancer's behaviour is disruptive to class management and/or instruction etc, our studio policy is to communicate directly with you/parent, and facilitate a solution to the behaviour. If this does not address the concern, we may not be able to have the student in the class.

If I/my dancer is unwell, I/they will not attend class.

I agree to:
•There are no refunds
•There is no credit for missed classes
•Have strong attendance to class- For the Winter Showcase term, I will miss no more than 2 classes out of the entire term.
-Be punctual.
•I will pay my costume fee as soon as it is posted if I am in the showcase.
• Be prepared with the proper CLEAN footwear and hair for the style, water, etc.
• to ask permission from the instructor if I wish to film a segment of the class.
•Be kind and inclusive to all
•Support my fellow dancers and will be willing to share knowledge and gain knowledge from the dancers around me
•Be respectful of my instructor
• If I have feedback about a teacher/another student/class management issues, etc., I will bring it up directly with staff at an appropriate time so the studio can address it directly. Feedback is so vital to our studio health, give us a chance to address your concern :)
