OUR SUMMER SCHEDULE IS JULY & AUGUST AND CLASS PACKAGES MUST BE PURCHASED FOR SUMMER CLASSES.

I authorize Fusion Studios:Dance Art Music, LLC (hereinafter "Fusion Studios") to perform scheduled or periodic electronic funds, transfer debits and/or credits from my account information provided and stored on my child's DanceStudio-Pro account for payments due or when applicable, apply electronic funds transfer credits to the same. I understand the dollar amount can vary depending on studio purchases.

I also acknowledge that I provided by debit/credit card information in person and authorize Fusion Studios to store this information on my child’s DanceStudio-Pro account to use the card I provided for future payments/autopay charges.

For accounting purposes, all electronic debits will be reflected in the monthly bank statement that corresponds with the credit/debit card information I provided in person.

I am aware that if a class has 2-3 kids, class will be 45 minutes. I am aware that if my child is the only one in class, class will be 30 minutes.

Some months during the year have 5 weeks of dance classes. Some months have three (Thanksgiving, Winter Break). No additional charge is applied when a fifth lesson occurs, nor is there a reduction when fewer than four classes are held due to a major holiday or absences. However, with months that have less than three classes, we will offer a make-up class at no additional charge.

Any students picked up later than 5 minutes after the last class of the day will be charged a Babysitting Fee. $1 will be charged, per minute - to the billing information on file after courtesy minutes are up.

I agree to be signed up to Fusion Studios periodic monthly email communications. I acknowledge that I will be signed up to receive 6-8 emails a year. I agree to stay subscribed to the communications for as long as my child is dancing to ensure that I do not miss any important information regarding studio happenings.

You have until the 15th of the month to make any changes to autopay set for the upcoming month.
Any changes made after the 15th will be applied to the following month.
YOU MUST EMAIL IN TO THE LOCATION YOUR CHILD TAKES CLASSES TO MAKE ANY CHANGES OR CANCELLATIONS TO YOUR UPCOMING AUTO-PAYS. Make ups will expire within 4 weeks of the missed class or if a student drops the class, whichever happens first. No Exceptions, No Refunds. Credits are non-transferrable.

$50 registration fee and first month's tuition upon registering is non-refundable and non-creditable. This holds your dancer's spot in their class. Registration fee is for the September-June season. If a student un-enrolls during a season and decides to re-enroll, the registration fee will be auto-charged again.

If I am signing up for a Holiday Show or Recital Class, I agree that my dancer will participate and pay the fees associated with those performances.

In checking the box below, I understand the nature of this activity in which I and/or my child shall participate. Therefore, I confirm and warrant that I and /or my child are in good health. However, if I have any doubts, I acknowledge that I was advised consult with a physician. I will in no way hold Fusion Studios: Dance Art Music, LLC (hereinafter “FUSION STUDIOS" or "Fusion") owners, employees, instructors, contractors, designees, or volunteers responsible for any accidents, personal injuries, illness, loss of or damage to personal property, which may occur at the studio or any other designated venue where Fusion students may perform. I, on behalf of myself and my child, do hereby Release, Waive, Discharge and Covenant not to sue Fusion’s owners, employees, instructors, contractors, designees, or volunteers from all liability for any loss or damage, and any claim or demands therefore on account of injury to the person or property or resulting in death, whether caused by the negligence or otherwise, while in, upon, or about the premises or any facilities or any other designated venue where Fusion students may perform or equipment, therein. Each student may decline to participate in any activity. It is incumbent on the Parent to inform the instructor of any physical limitations the student might have. I expressly agree on behalf of myself and my child that the foregoing Release, Waiver and Indemnity Agreement is intended to be as broad and inclusive as is permitted by the law of the State of California and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

Rental Agreement:
Renter assumes liability for injury of any persons they invite to the space, including but not limited to: injury to students in the renter’s class, injury to performers or collaborators in the rehearsal. Fusion Studios is not responsible for any loss, broken or stolen personal items.

I, the undersigned, on my behalf and on behalf of my child named below, hereby acknowledge that my
child and I desire to voluntarily participate in dance class held at the Fusion Studios: Dance Art Music, LLC (hereinafter “FUSION STUDIOS"). I fully understand and acknowledge that participation in the FUSION STUDIOS exposes me and/or my child to the risk of personal injury, death, communicable diseases, illnesses, viruses, or property damage. I FURTHER UNDERSTAND AND ACKNOWLEDGE THAT AN INHERENT RISK OF EXPOSURE TO COVID-19 EXISTS IN ANY PUBLIC PLACE WHERE PEOPLE ARE PRESENT. I EXPRESSLY AGREE TO ACCEPT AND ASSUME ANY SUCH RISKS ON BEHALF OF MYSELF AND MY CHILD.

I hereby release, discharge, and agree not to sue the Fusion Studios: Dance Art Music, LLC (hereinafter “FUSION STUDIOS") or any of its officers, employees, agents and volunteers for any injury, death or
damage to person or property, financial duress, arising out of, or in connection with, my and/or my child’s participation in the FUSION STUDIOS from whatever cause, including the active or passive negligence of the FUSION STUDIOS, its officers, employees, agents and/or volunteers or any other participants in the FUSION STUDIOS. In consideration for me and my child being permitted to participate in the FUSION STUDIOS, I hereby agree, for myself, my heirs, administrators, executors, and assigns, that I shall indemnify and hold harmless the FUSION STUDIOS and its officers, employees, agents, and volunteers from any and all claims, demands actions or suits arising out of or in connection with my and/or my child’s participation in the FUSION STUDIOS.

My child is physically able to participate in FUSION STUDIOS classes, events, etc. I acknowledge that NO MEDICAL INSURANCE IS PROVIDED by the FUSION STUDIOS for FUSION STUDIOS classes. I authorize the FUSION STUDIOS to provide or cause to be provided such medical treatment as may be necessary or appropriate if any injury occurs to me and/or my child while participating in the FUSION STUDIOS classes. I agree to pay for any medical treatment that I and/or my child may require while involved in the FUSION STUDIOS and not hold FUSION STUDIOS liable.

I hereby consent to the use of photographs depicting me and/or my child in FUSION STUDIOS printed materials and/or website.

I HAVE CAREFULLY READ THIS WAIVER, RELEASE, HOLD HARMLESS AND AGREEMENT NOT TO SUE AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT IT IS A FULL RELEASE OF ALL LIABILITY AND SIGN IT OF MY OWN FREE WILL. BY CHECKING THE BOX BELOW, I HAVE READ, UNDERSTAND, AND AGREE TO ABIDE BY ALL OF THE RULES, POLICIES, LIABILITY WAIVER AND PAYMENTS SET FORTH IN THIS CONTRACT. I UNDERSTAND I HAVE TO BE 18 YEARS OF AGE OR OLDER TO CHECK THE BOX BELOW WHICH IS SIGNIFYING MY SIGNATURE OF THIS REGISTRATION/LIABILITY/POLICY CONTRACT.
