Acknowledgment of Risk:
I understand that participating in classes involves risks, including but not limited to, falls, injuries, and other physical harm. I acknowledge that I am voluntarily participating in this activity and assume all risks associated with it.
Health and Fitness:
I confirm that I am in good health and physically fit to participate in dance classes. I will inform the instructor of any medical conditions or injuries that may affect my ability to participate.
Behavior:
I agree to follow the rules and guidelines set by the ACTiv Elite Performers. I will show respect to instructors, staff, and fellow students. I understand that failure to adhere to these guidelines may result in dismissal from the class without a refund.
Privacy:
I understand that the dance school may take photographs or videos during classes and events. I give permission for my image to be used for promotional purposes, including on the school's website and social media. If I do not wish to be photographed or recorded, I will inform the school in writing.
Release of Liability:
I hereby release and hold harmless ACTiv Elite Performers, its instructors, staff, and facilities from any and all liability, claims, demands, and causes of action arising from my participation in dance classes.
Agreement:
By registring and paying my fee's, I acknowledge that I have read and understood this waiver, and I agree to its terms.
