I am aware that dance training and the athletic exercises associated with it place unusual stress on the body and carry a risk of physical injury. On behalf of my child and myself, (and if I am no longer a minor, on my own behalf), I assume the risk and agree that the Maple Leaf School of Ballet shall not be liable in any way for injuries sustained during attendance, dance classes, or any related functions. I understand that dance training involves some appropriate minor hands-on adjustments of the student's body by the instructor. If a student feels uncomfortable with any touching by a representative of the School then they are to report it to the School Administration as soon as possible. I further acknowledge that neither the Maple Leaf School of Ballet nor its instructors are responsible for any loss of or damage to any of the student's personal property while on School Property, or at any functions or events that the School is participating within.

I hereby authorize the Maple Leaf School of Ballet and its agents to record the student's picture, voice, and any other renderings of image or persona, on photographs, films, and tapes, and/all future means of recording and/or capturing of student’s likeness, to edit these recordings/media at its discretion, and to incorporate these recordings into movie, and sound films and/or digital and/or all future forms of media, for use at the discretion of the School. I also give my permission for the Maple Leaf School of Ballet to use and license others to use these materials for publicity, advertising, and sales promotion, and to use the student's name, likeness, voice, and biographic or other information in connection with them. I acknowledge that no promises of compensation are made by the Maple Leaf School of Ballet for such use.

Registration and the first and last month's payment must be completed before students are permitted to participate in any classes.

The following forms of payment are accepted: e-transfer, cash, cheque or electronically through Dance Studio Pro and Stripe. As a small business, we encourage and are grateful for payments made via e-transfer that help reduce the transaction fees involved with processing payments using credit and/or debit cards. We do, however, require all students to enroll in auto-pay with Dance Studio Pro, as a back-up form of payment. Please email us at info@mapleleafballet.ca to specify your preferred form of payment if it is not through the Dance Studio Pro. Please note, that if you select a credit card as your payment method, you are providing Maple Leaf School of Ballet permission to process payment on that card as needed.

A registration fee will be charged to your Dance Studio Pro account on the date you register. If you'd like to pay via e-transfer, the payment must be received before the time of registration so that the administration can manually post the payment to your Dance Studio Pro account which allows the Software to continue the enrolment process and ensure the students placement within the selected class(es).

Registration fees are non-refundable.

There is an additional fee for participating in the Nutcracker. This fee covers the additional rehearsal time, costuming, and annual improvements to the production. Once participation is confirmed, this $100 fee is non-refundable. It will be posted to your Dance Studio Pro Account and is due November 1st.

$50 Costume rental fee per level is due March 1st, 2025 for our Jubilee show in June. Performance fees are posted when the student's participation is confirmed for any given performance and, are non-refundable.

Tuition fees will be posted to your Dance Studio Pro account on the 1st of each month, starting on the first day of classes.

Families who wish to pay tuition and/or costume fees by e-transfer may do so by e-transferring their payment BEFORE the payment due date. For example, if your September tuition is due September 1, please e-transfer your September fees by August 31.

If you wish to pay for the dance season as a lump sum upfront via e-transfer, please contact info@mapleleafballet.ca once you have completed your registration and paid your registration fee.

A $25.00 NSF fee will be charged for any declined payments.

Withdrawals: Students may withdraw before November 1st, 2024, and without penalty (forfeiting the Registration fee). Notice of withdrawal must be provided via email to info@mapleleafballet.ca. After November 1st, 2024, one month's notice is required for withdrawal. No refunds will be given for withdrawal from classes after December 31, 2024, and any outstanding fees and the remaining tuition balance must be paid at the time of withdrawal.

Classes may be cancelled or moved due to low registration. Should this occur, no refunds will be given for previously paid fees; however, no new tuition payments will occur unless the dancer is still participating in the changed class(es) or transferred to a new class. If the student has pre-paid their tuition for the year, then their credit will remain in the student's account and be applied to the changed class(es). If there is a credit at the end of the year due to the class changes, the credit may either be carried forward and applied to future services, or it may be refunded, depending on decisions made by both the client and the school. However, if the cost of the changed classes is more than the cost of the original classes registered, no additional tuition will be charged to the client.

Students registered for more than one class will receive a discount with each class the student is registered in. The more classes the student is enrolled in, the greater the discount is applied to the Dance Studio Pro Account.
