Elite School of Dance Waiver

Registration, Tuition, and Fee's:
The stated, as a guardian of the listed student(s), makes application on his/her behalf for acceptance into classes at Elite School of Dance. I understand a $30 NON-REFUNDABLE registration fee must be paid per student as part of enrollment.
Summer Classes/Camps: Tuition for Summer classes/camps is due at the time of registration. Students will not be registered for summer classes until the tuition is paid to hold their spot.
As Guardian of the registered student, you are responsible for all tuition for each month from September 1st or from the time of enrollment through the recital date, unless the receptionist is notified to drop the class from your account. Notification to the office is required to drop a class even if the student does not attend classes for several weeks . The $30 registration fee is required to be paid again if a student withdraws at any time and returns thereafter.
Tuition is due the first class of the month. If tuition payment is not received by the 15th of each month, a $15.00 late fee will be added to your account. If the tuition is not received by the end of the month, the student will be only allowed to observe class and will not be allowed to participate. We will allow the student to call his/her parent to rectify the situation. After two (2) consecutive months of non-payment, the student cannot enter the class(es) and will be required to repay the $30 registration fee upon return.

Termination Guidelines:
A 30 day in advance emailed notice will only be accepted by the Elite School of Dance office to terminate enrollment before the end of the season. Verbal or telephone notice to a faculty member or to the office is not valid .
To terminate enrollment, drop a class, or make changes, it must take place either at the end of a month with ALL TUITION PAID IN FULL through the end of that month, even if the student does not attend classes through the month.

Hold Harmless Agreement:
In consideration for my child being permitted to participate in Elite School of Dance, program (referred to as “the Studio”), I/We parent(s) and/or legal guardian(s) of the dancers listed on my account, agree to the following provisions:
Acknowledgment of Risks: I/We understand that there are numerous risks associated with participating in dance activities, including those risks present during classes and activities, on the premises before, during, and after various classes and events, as well as in travel to and from the Studio and various venues of performance. Some of the specific risks include the placement of unusual stresses on the body, falls and tripping which may lead to accidents resulting in, but not limited to, mild to severe bodily injury. I/ We understand that the Studio cannot be responsible for any injuries or damages experienced by my/our child during her/his participation in such activities.
Medical Care: In an emergency situation, when parental permission is not available, I/ We hereby grant and give my/our permission for a staff member at the Studio to seek emergency medical treatment for my /our child. In my/our absence or inability to communicate with emergency and hospital personnel, I/ We hereby grant the Studio authority to release for the purposes of providing medical treatment, my/our child to the care of medical personnel or physicians as the Studio determines as reasonably appropriate.
Media Relations: By signing the Agreement and Releases, I/We consent and give permission to the Studio, and those acting under its authority, to use the name or likeness of my child for the purpose of marketing the studio to the media.
Release, Covenant Not to Sue, Waiver of Liability and Indemnification Provisions Parents (of Minors): I/We have read and clearly understand the terms of this Agreement. I/We give my/our permission for my/our child
to participate in all Studio activities, including those described above. I/ We also release and hold harmless the Studio , its officers, directors, agents and employees, and those acting under its authority, from all actions, claims and liabilities relating to my/our child’s participation in any and all programs. I further agree to indemnify and hold harmless and defend Directors Tony and Heidi Gamlieli, Elite School of Dance and it’s teachers, officers, agents, and employees from injuries, damages and losses sustained by me or my child arising out of, connected with, or in any way associated with the activities of the program(s). I have read and fully understand the above program details and waiver and release of all claims.

Exceptions:
The policies of Elite School of Dance will not change or exceptions cannot be made by Elite School of Dance staff , as these policies are set in place to ensure professional operation of Elite School of Dance.
We reserve the right to refuse service and terminate enrollment .
