Liability Waiver / Assumption of Risk
I hereby agree to release Lake Oswego Academy of Dance (hereinafter referred to as “LOAD”) and hold LOAD employees, instructors, subsidiaries, parent corporations and all affiliated entities harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury, including COVID 19, arising out of active participation in dance and related activities on behalf of the participant; I agree that we are voluntarily participating in activities and use of said facilities, premises (including the shoppe, sidewalk and parking lot), and designated LOAD event venues. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury.

Payment Policy
LAKE OSWEGO ACADEMY OF DANCE has my permission to charge all fees associated with my account for the current dance season on the credit card submitted. I understand it is my responsibility to continually update my credit card information to keep my account in good standing.

Payments received after the 10th will be charged an additional $15.00 late fee. A $25.00 fee will be charged for all returned checks. Tuition is non-refundable. LOAD reserves the right to cancel any class due to insufficient enrollment, in which case a prorated refund will be issued.

Photo Release Waiver
I am aware that photos, videos, and audio recordings of my child may be captured at the studio and studio related events. I grant permission to Lake Oswego Academy of Dance to use my child's name, voice, and likeness in any form of media, without additional compensation or consideration, notification, or permission, unless prohibited by applicable law. I waive any right that I or my child may have to inspect or approve any finished product including any form of media, advertising and/or other materials

I have signed this document/agreed by checking the box voluntarily and of my own free will in exchange for the privilege of participation.

I am aware that dropping a class or withdrawing from the studio must be done by contacting the studio office, on or before the 20th of the month prior to the next billing period. If the request is made to drop or withdraw after the 20th, the account will still be charged the full amount of the following month's tuition.
