Photo Release:
I authorize Dance Collaborative (DanceCollab), its agents, employees, sponsors, or other authorized parties to include me or my child in photographs, videos, audio or other recordings for any lawful purpose; including, without limitation: education, public information, marketing, fundraising, or in connection with any release of information to the media. I hereby grant to DanceCollab all rights to use and publish such photographs, videos, audios or other recordings and to alter same without restrictions and without prior examination or approval by me. I understand that I have no right to compensation in connection with this consent.

Liability Release:
I understand that dance involves physical contact, that serious accidents occasionally occur, and that participants occasionally sustain serious personal injuries and/or property damage. Knowing the risks, I, on behalf of myself, my child, and any heirs or assigns thereof, hereby release and hold harmless DanceCollab and its agents, employees, sponsors, or other authorized parties from any and all liability resulting in connection with my child's participation in DanceCollab programs. In addition, I acknowledge that DanceCollab and its employees are not medical providers, and that their programing is presented for informational purposes only, and is not intended to substitute for professional medical advice. Lastly, I am aware that dance also requires physical contact in order to correct alignment, demonstrate proper placement, and help students succeed and do hereby give DanceCollab and its agents permission to appropriately address the aforementioned situations.

COVID-19 Release:
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child and I may be exposed to or infected by COVID-19 by attending dance classes with Dance Collaborative and that such exposure or infection may result in personal injury, illness, permanent disability, or death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including but not limited to, Dance Collaborative, participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child or myself including, but not limited to illness, damage, loss, claim, liability, or expense, of any kind. On my behalf, and on behalf of my child, I hereby release, covenant not to sue Dance Collaborative, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Dance Collaborative and its employees.
